摘要
目的探讨孕妇宫颈沙眼衣原体感染与不良妊娠结局的关系。方法采用计算机检索1997—2017年中国期刊全文数据库(CNKI)、中文科技期刊全文数据库(维普)、万方-数字化期刊全文库,并辅以文献追溯方法,收集国内公开发表的有关宫颈沙眼衣原体感染与不良妊娠结局关系的相关文献。应用Rev Man 5.3.5软件进行Meta分析。结果将15个研究结果加权合并,累计宫颈沙眼衣原体阳性者903例,阴性者3 682例。Meta分析结果显示胎膜早破(OR=7.38,95%CI:4.41~12.36)、早产(OR=4.17,95%CI:3.14~5.54)、低出生体重儿(OR=4.12,95%CI:2.92~5.80)、新生儿肺炎(OR=8.07,95%CI:4.52~14.41)、新生儿结膜炎(OR=26.69,95%CI:9.07~78.49)、新生儿感染(OR=22.58,95%CI:11.45~44.56)、产褥病率(OR=5.58,95%CI:2.90~10.76)和宫颈沙眼衣原体感染有显著相关性。结论目前国内孕妇宫颈沙眼衣原体感染与不良妊娠结局发生有一定关系,需引起临床重视。
Objective To discuss the relationship between cervical chlamydia trachomatis （CT） infection and adverse pregnancy outcomes. Methods After a recursive literature searching for the published reports between 1997 and 2017 in CNKI database, VIP database and Wanfang database, literatures about the relationship between cervical CT infection and adverse pregnancy outcomes were collected. The Cochrane Collaboration＇s software Rev Man 5.3.5 was used to analyze the data. Results Results of 15 studies were weightedly merged. A total of 903 cervical CT positive cases and 3 682 cervical CT negative controls were collected. The incidence of premature rupture of membranes （OR = 7.38,95 %CI ： 4.41 - 12.36 ）, premature delivery （ OR = 4.17,95% CI ： 3.14 - 5.54）, low birth weight infant （OR = 4.12,95% CI ： 2.92 - 5.80）, neonatal pneumonia （OR = 8.07, 95 % CI ： 4.52 - 14.41 ）, neonatal conjunctivitis （OR =26.69,95% CI ： 9.07- 78.49）, neonatal infection （OR =22.58,95% CI ： 11.45-44.56） and puerperal morbidity （OR = 5.58,95% CI ： 2.90- 10.76） between CT positive group and negative group were significantly correlated with CT infection. Conclusion Cervical CT infection in pregnant women at present is obviously associated with the incidence of adverse pregnancy outcomes, which needs attention from medical staff.
