摘要
目的:探讨参芪健胃汤联合替吉奥胶囊治疗老年晚期胃癌气血两虚证的临床疗效及对血清MMP-2、MMP-9和Treg细胞的影响。方法:收集2014年1月至2017年1月入院的100例晚期胃癌气血两虚证老年患者作为研究对象,随机分为对照组和试验组,对照组患者给予替吉奥胶囊口服,试验组在对照组治疗基础上加服参芪健胃汤,比较两组患者中医证候评分、临床疗效、基质金属蛋白酶-2(matrix metalloproteinase-2,MMP-2)、基质金属蛋白酶-9(matrix metalloproteinase-9,MMP-9)、Treg细胞活力与生存质量。结果:试验组患者治疗后各维度中医证候评分均显著低于对照组,两组比较,差异有统计学意义(P<0.01);试验组有效率显著高于对照组,两组比较,差异有统计学意义(P<0.01);治疗后试验组MMP-2与MMP-9水平均显著低于对照组,CD_4^+CD_(25)^+T与CD_4^+CD_(25)^-T水平均显著高于对照组,差异均有统计学意义(P<0.01);试验组躯体功能评分显著高于对照组,疲倦、恶心与呕吐、疼痛、食欲减退与腹泻评分均显著低于对照组,两组比较,差异均有统计学意义(P<0.01)。结论:参芪健胃汤联合替吉奥胶囊治疗老年晚期胃癌气血两虚证疗效显著,可降低MMP-2、MMP-9水平,提高CD_4^+CD_(25)^+T与CD_4^+CD_(25)^-T水平。
Objective： To investigate the effect of Shenqi Jianwei decoction combined with Tiggio in the treatment of senile gastric cancer with Qi and blood deficiency syndrome and its effect on serum matrix metalloproteinase-2（ MMP-2）,matrix metalloproteinase-9（ MMP-9） and Treg cells. Methods： 100 patients with advanced gastric cancer were enrolled in this study from January 2014 to January 2017. They were randomly divided into two groups. The patients in the control group were given Tiggio. The patients in the experimental group were treated with Shenqi Jianwei decoction. Compared the two groups of patients with TCM syndrome score,the overall treatment effect,metalloproteinase,Treg cell viability and quality of life. Results： Compared with the control group,the scores of TCM syndromes of the experimental group were significantly lower than those of the control group（ P〈 0. 01）. The effective rate was significantly higher than that of the control group（ P 〈0. 01）. After treatment,the levels of MMP-2 and MMP-9 were significantly lower than those of the control group. The levels of CD4＋CD25＋T and CD4＋CD25-T in the experimental group were significantly higher than those in the control group（ P 〈0. 01）. The body function scores of the experimental group were significantly higher than those of the control group,but fatigue,nausea and vomiting,pain,loss of appetite and diarrhea scores were significantly lower than those in the control group（ P〈 0. 01）. Conclusion： Shenqi Jianwei decoction combined with Tiggio capsule in the treatment of advanced gastric cancer with Qi and blood deficiency has significant effect and it could decrease the level of MMP-2,MMP-9 and increase the levels of CD4＋CD25＋T and CD4＋CD25-T.
