摘要
目的研究分析组织病理学诊断子宫颈病变的临床应用价值。方法选取2016年5月至2017年5月行子宫颈癌筛查的女性共1000例为研究对象,将液基细胞学(TCT)检查结果与组织病理学结果进行对比分析,计算TCT诊断的准确率。结果 1000例受检者组织病理学诊断结果为正常642例、炎症135例、宫颈上皮内瘤变(CIN)Ⅰ级86例、CINⅡ级65例、CINⅢ级48例、浸润癌13例、其他11例;TCT诊断结果为阴性633例、无明确诊断意义的不典型鳞状上皮细胞(ASC-US)95例、不典型鳞状细胞(ASC)67例、不除外高度上皮内病变的不典型鳞状细胞(ASC-H)78例、低度鳞状上皮内病变(LSIL)61例、高度鳞状上皮内病变(HSIL)34例、不典型腺上皮细胞(AGC)23例、AGC倾向瘤变9例,阳性检出率为36.7%。TCT检查的准确度为99.1%,灵敏度为50.0%,特异度为63.9%。结论 TCT诊断子宫颈病变具有较高的准确性,利于对宫颈癌前病变与宫颈癌的筛出,但仍需采取进一步的组织病理学诊断进行确诊并采取相应的治疗措施,临床应用价值高。
Objective To study the clinical value of histopathological diagnosis of cervical lesions. Methods A total of 1000 women who underwent cervical cancer screening between May 2016 and May 2017 were selected as the study objects. The results of TCT and histopathological results were compared and analyzed to calculate the diagnosis of TCT The accuracy rate. Results The results of histopathological diagnosis in 1000 cases were 642 cases of normal, 135 cases of inflammation, 86 cases of grade Ⅰ cervical CIN, 65 cases of CIN Ⅱ grade, 48 cases of CIN Ⅲ grade, 13 cases of invasive carcinoma, And the other 11 cases. The negative results of TCT were 633 cases, 95 cases of atypical squamous cell(ASC-US) without definite diagnosis, atypical squamous cells(ASC) in 67 cases of atypical squamous cells, cannot exclude high-grade intraepithelial neoplasia(ASC-H), 78 cases of low-grade squamous intraepithelial lesion(LSIL), 61 cases of high grade squamous intraepithelial lesion(HSIL), 34 cases of atypical glandular cells(AGC) in 9 cases of 23 cases, AGC tendency of tumors, the positive rate was 36.7%. The accuracy of TCT was 99.1%, the sensitivity was 50.0% and the specificity was 63.9%. Conclusion The diagnosis of cervical lesions by TCT has high accuracy, which is beneficial to the screening of cervical precancerous lesions and cervical cancer, but still need to take further histopathological diagnosis to confirm and take corresponding treatment measures, and its clinical value is high.
