活血通络方对膜性肾病大鼠肾功能的影响及对肾组织IFN-γ、VEGF干预机制的研究 被引量:7

摘要 目的:探讨活血通络方治疗膜性肾病的疗效,为临床治疗膜性肾病提出新的方法和途径。方法:将90只SD雄性大鼠,适应性喂养7天后随机选取15只为空白对照组,其余75只参照改良Border法进行造模,造模成功后将大鼠随机分为模型对照组、贝那普利组、活血通络方5.04、10.08、20.16g/kg组。空白对照组与模型对照组大鼠给予蒸馏水,贝那普利组给予盐酸贝那普利片(10mg/kg/d),治疗组给予活血通络方。在给药的第4周末检测大鼠血总蛋白(TP)、血白蛋白(ALB)、24小时尿蛋白定量(24hUTP)、γ干扰素(IFN-γ)、血管内皮生长因子(VEGF)及IFN-γmRNA、VEGFmRNA表达。总胆固醇(TC)、甘油三酯(TG)、尿素氮(BUN)、血肌酐(Scr)、血尿酸(UA)的水平;观察大鼠肾脏病理形态学变化;免疫组化及Real-time PCR检测肾组织细胞因子的表达。结果:大鼠造模期间,共有3只死亡。造模4周后,造模组大鼠24h尿蛋白定量较空白对照组明显升高;治疗结束后,与空白对照组相比,模型对照组TP、ALB显著下降,24h-UTP、TC及TG显著升高,与模型对照组相比,活血通络方治疗各组与贝那普利组TP、ALB均不同程度升高,TC、TG均明显下降,活血通络方5.04、10.08、20.16g/kg/d组与贝那普利组相比,差异不明显,各组BUN、Scr水平无明显差异;治疗结束后与模型对照组比较,贝那普利组及活血通络方5.04、10.08、20.16g/kg/d组IFN-γmRNA表达下降,VEGF mRNA表达显著上升。结论:活血通络方可明显升高膜性肾病大鼠血总蛋白、血白蛋白水平,显著降低膜性肾病大鼠24小时尿蛋白定量,降低血TC、TG水平,下调膜性肾病大鼠肾组织IFN-γmRNA,上调VEGF mRNA,具有保护肾功能的作用。 Objective: To explore the clinical effect of Huoxuetongluo Recipe on membranous nephropathy,and to propose new methods and approaches for the clinical treatment of membranous nephropathy. Methods: Ninety SD rats were randomly divided into control group and control group. The rats were randomly divided into model control group and control group. The rats were randomly divided into model control group,Western medicine treatment group,Huoxuetongluo 5. 04 g/kg/d group,Huoxuetongluo 10. 08 g/kg/dmiddle dose group,Huoxuetongluo 20.16 g/kg/d group. The rats in the blank control group and the model control group were given distilled water and the western medicine control group was given benazepril hydrochloride tablets. The treatment group was given Huoxuetongluo recipe. Total Cholesterol(TC),serum albumin(ALB),24-hour urine protein(24 h-UTP),total cholesterol(TC),triglyceride(TG),urea nitrogen(BUN),serum creatinine(Scr)and serum uric acid(UA) were measured. The changes of renal pathological changes were observed. Immunohistochemistry and Real-time PCR were used to detect the expression of cytokine interferon γ(IFN-Endothelial growth factor(VEGF),IFN-γ mRNA and VEGF mRNA.Results: During the rat model,a total of three died.(P〈0. 01). Compared with the blank control group,the model group was significantly higher than that in the control group(P〈0. 01). Compared with the blank control group,the model group was significantly higher than that in the blank control group(P〈0. 05). Compared with the model control group,the levels of TP and ALB were significantly higher in the treatment group and the western medicine group than that in the control group(P〈0. 05),and the difference was statistically significant(P〈0. 05). There was no significant difference between the two groups(P〈0. 05). There was no significant difference between the two groups(P〈0. 05).
