某三甲医院2016年血培养标本病原菌分布及耐药性分析 被引量:4

摘要 目的:监测和分析某医院2016年血培养中的主要病原菌及其耐药性,为临床合理用药提供参考。方法:对某医院2016年1月-2016年12月的病人血培养标本结果进行统计分析。结果:1104份血培养标本共分离出病原菌147株,阳性率为13.32%,其中革兰阳性菌96株,占65.31%;革兰阴性菌48株,占32.65%;真菌3株,占2.04%;分离的病原菌中前5位依次为凝固酶阴性葡萄球菌(CNS)占44.22%、金黄色葡萄球菌(SAU)占9.52%、大肠埃希菌占8.84%、肺炎克雷伯菌占6.12%和嗜麦芽窄食单胞菌占3.40%。葡萄球菌中耐甲氧西林凝固酶阴性葡萄球菌(MRCNS)和耐甲氧西林金黄色葡萄球菌(MRSA)分别占CNS和SAU的80.00%和28.57%,对达托霉素、万古霉素、利奈唑胺的敏感率为100.00%;大肠埃希菌和肺炎克雷伯菌中超广谱?-内酰胺酶(ESBLs)的检出率分别为76.92%和22.22%,对阿米卡星、碳青酶烯类、哌拉西林/他唑巴坦、阿莫西林/克拉维酸和替卡西林/棒酸的敏感度达100.00%。结论:血培养病原菌以革兰阳性菌为主,对常用抗菌药物耐药严重,应合理应用抗菌药物。 Objective:To investigate the distribution and antimicrobial resistance of pathogens isolated from blood culture in 2016 and provide basis for clinical treatment. Methods:Date of pathogens from blood specimens of patients in a hospital between January 2016 and December 2016 were analyzed statistically. Results:A total of 147 strains of pathogens were isolated from 1104 blood cultures with the positive rate was 13.32%, including 96(65.31%) strains of gram-positive bacteria, 48(32.65%) strains of gram-negative bacteria, and 3(2.04%) strains of fungi; the coagulase-negative Staphylococcus, Staphylococcus aureus, Escherichia coli, Klebsiella pneumoniae and Stenotrophomonas maltophilia were ranked the top five species of pathogens, accounting for 44.22%, 9.52%, 8.84%, 6.12%, 3.40%, respectively. Prevalence of MRCNS and MRSA among CNS and S.aureus were 80.00% and 28.57%,respectively. Staphylococcus had 100.00% sensitivity to daptomycin,vancomycin and linezolid; the detectable rates of Escherichia coli, Klebsiella pneumoniae of ESBLs were 76.92% and 22.22%, and the strains had 100.00% sensitivity to amikacin, carbapenems, piperacillin-tazobactam, amoxicillin-clavulanic acid and ticarcillin-clavulanic acid. Conclusion:The gram-positive bacteria are dominant among the p athogens isolated from blood culture, the pathogens are highly drug-resistant. We should enhance the drug resistance monitoring so as to reduce the drug resistant strains.
