摘要
目的探讨可溶性fms样酪氨酸激酶1(sFlt-1)、淀粉酶(AMY)及脂肪酶(LIP)对急性胰腺炎(AP)的早期诊断价值。方法纳入AP患者70例,分为轻度组(n=43)和中重度组(n=27),另选取同期30例急腹症(非AP)患者作为对照组。常规检测研究对象的血清AMY、LIP水平,采用酶联免疫吸附法(ELISA)检测血清sFlt-1水平。结果 3组的血清sFlt-1、AMY及LIP水平比较,差异有统计学意义(P均<0.01);进一步两两比较显示,中重度组血清sFlt-1、AMY、LIP水平均明显高于轻度组和对照组(P均<0.01),轻度组均明显高于对照组(P均<0.01)。受试者工作特征曲线(ROC曲线)分析显示,3个指标联合检测的曲线下面积(AUC)均明显大于sFlt-1、LIP单独检测(z=2.034,P=0.042;z=2.456,P=0.014);3个指标联合检测的灵敏度及特异度均优于各指标单独检测(P均<0.05)。结论血清sFlt-1水平对于AP的早期诊断和AP严重程度评估有一定的临床参考价值,血清sFlt-1、AMY和LIP联合检测可提高AP早期诊断的灵敏度和特异度。
Objective This paper intends to investigate the value of early diagnosis of acute pancreatitis（AP）by soluble fms-like tyrosine kinase 1（sFlt-1）,amylase（AMY）and lipase（LIP）.Methods Seventy patients with AP were enrolled and divided into the mild group（MAP group,n=43）and the moderately severe group（MSAP group,n=27）.30 cases of acute abdomen patients with non-AP in the same period were selected as the control group.The serum amylase and the lipase content were routinely tested,while the serum sFlt-1 was measured by using enzyme-linked immunosorbent assay.Results The levels of serum sFlt-1,AMY,and LIP of the three groups were significantly different（all P〈0.01）.The serum sFlt-1,AMY,and LIP levels of the MSAP group were significantly higher than those of the MAP group and the control group（all P〈0.01）.The serum sFlt-1,AMY,and LIP levels of the MAP group were significantly higher than those of the control group（P〈0.01）.The ROC curve analysis showed that the area under the curve（AUC）of the combined detection of sFlt-1,AMY,and LIP indexes was significantly higher than the sFlt-1 and the LIP index alone（z=2.034,P=0.042;z=2.456,P=0.014）.The sensitivity and specificity of combined detection of the three indexes were better than those of each index alone（all P〈0.05）.Conclusions The serum sFlt-1 level has a certain clinical reference value in the early diagnosis and assessment of the severity for AP.The combined detection of sFlt-1,AMY,and LIP can improve the sensitivity and specificity of early diagnosis of AP.
