情景模拟训练对眼科局麻手术患者手术耐受程度的影响 被引量:14

摘要 目的探讨情景模拟训练对眼科局麻手术患者手术耐受程度的效果。方法选择2016年2~4月眼科局麻手术患者100例,随机分为对照组与观察组。对照组患者给予常规手术指导,观察组患者加入情景模拟训练内容。比较两组患者入院后第1天和术前2 h的血压、心率,焦虑自评量表(SAS)和抑郁自评量表(SDS)评分,以及借助视觉模拟评分(VAS)尺完成术中疼痛、恐惧和配合程度评分。结果两组患者各项指标评分在入院后第1天无明显差异(P>0.05),术前2 h时,观察组的收缩压(P<0.05)、舒张压(P<0.01)、心率(P<0.01)SAS和SDS评分均显著低于对照组,术中疼痛和恐惧评分也明显小于对照组,而术中配合程度明显高于对照组。讨论情景模拟训练应用于眼科局科手术患者,能够有效减少患者生理和心理应激反应,提高其对手术的耐受程度。 Objective To explore the effects of pre-operative scene simulation training in ophthalmic patients who are going to receive surgery under local anesthesia. Methods 100 patients undergoing ophthalmic surgery with local anesthesia were included and randomly divided into control and observation group. Patients in control group received routine ophthalmic guidance,while those in observation group were given additional scene simulation training. Blood pressure,heart rate,scores of self-rating anxiety scale( SAS) and self-rating depression scale( SDS) of the first day in the hospital and 2 hours before the operation were recorded and compared between two groups. Scores of pain,fear and coordination in the operation were evaluated by visual analogue scale and analyzed. Results There was no obvious difference of every index and score between two groups at the first day in the hospital. At 2 hours before the operation,systolic pressure,diastolic pressure,heart rate,scores of SAS and SDS were significantly lower in observation group than those in control group.Scores of pain and fear in the operation were obviously lower in observation group than those in control group,while score of coordination in the operation was significantly higher in observation group. Conclusions Application of scene simulation training in ophthalmic patients with surgery under local anesthesia could reduce patients' physiology and psychology reaction and improve the tolerance level of operation.
