摘要
目的探讨三维CT血管成像(3D-CTA)与三维脑血管造影(3D-DSA)检查在脑动脉显微夹闭术中的评估价值。方法选取2013年1月至2016年2月在我院治疗的疑似颅内动脉瘤患者92例,以手术结果为金标准,评价3D-CTA和3D-DSA诊断颅内动脉瘤的价值。结果 92例患者中,3D-CTA检查阳性74例,共诊断动脉瘤78个,3D-DSA检查阳性73例,共诊断动脉瘤76个;3D-CTA诊断颅内动脉瘤的灵敏度为97.33%,特异度为94.12%;3D-DSA诊断颅内动脉瘤的灵敏度为97.33%,特异度为100.00%;3D-CTA检测出的瘤体图像质量明显优于3D-DSA(P<0.05)。结论3D-CTA在诊断脑动脉瘤能力与3D-DSA相当,但对动脉瘤解剖结构和周边结构的显示要明显高于3D-DSA,提供的信息满足显微夹闭的需要。
Objective The evaluation value of three-dimensional CT angiography(3 D-CTA) and threedimensional digital subtraction angiography( 3 D-DSA) in microsurgical clipping of cerebral aneurysms was investigated. Methods A total of 92 patients with suspected intracranial aneurysms treated in our hospital from January 2013 to February 2016 were enrolled in the study. The surgical results were regarded as a gold standard to evaluate the value of 3 D-CTA and 3 D-DSA in the diagnosis of intracranial aneurysms. Results Among 92 cases,3 DCTA was positive in 74 cases and 78 aneurysms were diagnosed; 3 D-DSA was positive in 73 cases and 76 aneurysms were diagnosed. The sensitivity and specificity of 3 D-CTA in the diagnosis of intracranial aneurysms were 94. 12%and 97. 33%; the sensitivity and specificity of 3 D-DSA in the diagnosis of intracranial aneurysms were 97. 33% and100%; the image quality of tumor in 3 D-CTA detection was better than that of 3 D-DSA( P〈0. 05).Conclusion The ability of 3 D-CTA in diagnosing cerebral aneurysm is comparable to 3 D-DSA,but 3 D-CTA shows the better anatomic structure and peripheral structure than 3 D-DSA,which can provide the necessary information for the microsurgical clipping.
