摘要
目的分析系统性硬化症(systemic sclerosis,SSc)皮损中催乳素(prolactin,PRL)及催乳素受体(prolactin receptor,PRLr)表达与局部Th1/Th2细胞因子分泌模式的关联,同时分析皮损PRL水平与患者皮肤硬化修订Rodnan评分的相关性,探讨PRL在SSc患者皮损免疫调节方面的作用机制。方法收集我院治疗的28例SSc患者,采用免疫组化方法检测患者皮损和10例正常人皮肤组织中PRL、PRLr、IFN-γ、IL-4表达水平,并分析上述指标间的相关性。入选SSc患者进行皮肤硬化修订Rodnan评分,分析与皮损PRL表达的相关性。结果 PRL、PRLr表达于皮肤角质形成细胞、汗管腺上皮细胞、血管内皮细胞及浸润的淋巴细胞。SSc组PRL的表达高于正常对照组(P<0.05),PRLr的表达在2组间无差别(P>0.05)。IFN-γ、IL-4表达于浸润的淋巴细胞、皮肤角质形成细胞、血管内皮细胞。SSc组IL-4的表达高于正常对照组(P<0.05),IFN-γ在2组间的表达无差别(P>0.05)。SSc组中PRL与IL-4的表达正相关(r=0.693,P<0.001)。SSc皮损PRL表达与皮肤硬化修订Rodnan评分无明显关联(r=-0.09,P>0.05)。结论 SSc患者皮损表达PRL及PRLr,皮损局部存在Th紊乱模式,表现为Th2型细胞因子占优势,且高表达的PRL与Th2细胞因子正相关。PRL可能参与SSc皮损局部Th1/Th2细胞因子的免疫失衡,但与皮肤硬化程度无明显关联。
To investigate the immune regulation role of prolactin(PRL) in systemic sclerosis(SSc) skin lesions, total of 28 patients with SSc were recruited from the Second Affiliated Hospital of Fujian Medical University.Immunohistochemistry was used to detect the levels of PRL, prolactin receptor(PRLr), IFN-γ and IL-4 expression in skin biopsy specimens of patients with SSc and healthy control subjects. Modified Rodnan score was used to record the skin thickness of SSc patients. PRL, PRLr were detected in keratinocytes, epithelial cells of the sweat duct, vascular endothelial cells and perivascular lymphocytes of both groups. IFN-γ and IL-4 were detected in perivascular lymphocytes, keratinocytes, and vascular endothelial cells of both groups. Data showed that PRL expression was significantly higher in SSc skin lesions than that in normal skin tissues(P0.05); IL-4 expression was elevated in SSc skin lesions compared to normal skin tissues(P0.05). A positive correlation was found between PRL and IL-4 expression in SSc skin lesions(r=0.693, P0.001). There was no significant correlation between PRL expression and modified Rodnan score of SSc patients(r=-0.09, P0.05). In conclusion, PRL may be involved in the balance of Th1/Th2 in SSc skin lesions but not the skin thickness.
