摘要
目的:探讨哮喘急性期痰液细胞学分型与治疗方案及效果相关性。方法:选取2016年3月至2017年4月中山市横栏医院收治的急性发作哮喘患者98例作为观察组,选取同期体检健康自愿者50例作为对照组,观察组患者给予糖皮质激素冲击治疗,缓解后改用标准治疗方案,在患者急性期、缓解期分别诱导痰液作痰液细胞学分型检测和肺功能检查,比较哮喘急性发作期、缓解期和健康人群痰液细胞学特点,探究不同治疗方案对哮喘缓解期患者病情控制情况,分析痰液细胞学特点与肺功能相关性。结果:各组间中性粒细胞比较,差异均无统计学意义(P>0.05);嗜酸性粒细胞在哮喘急性期、哮喘缓解期、对照组中水平逐渐降低,组间比较,差异具有统计学意义(P<0.05);淋巴细胞在哮喘缓解期中显著低于哮喘急性期和对照组,差异具有统计学意义(P<0.05);巨噬细胞在哮喘急性期中显著低于哮喘缓解期和对照组,差异具有统计学意义(P<0.05);哮喘缓解期轻度和中度患者疾病控制效果比较,差异均无统计学意义(P>0.05)。中性粒细胞与肺功能无相关性,嗜酸性粒细胞、淋巴细胞与肺功能呈负相关性,巨噬细胞与肺功能呈正相关性。结论:痰液细胞学分型在哮喘急性期、稳定期中呈变化水平,与肺功能有一定相关性,标准化治疗能有效控制患者病情,诱导痰液细胞学检查可判定哮喘疾病严重程度,指导临床用药。
Objective To investigate the relationship between acute asthmatic sputum cytological classification and treatment regimen and effect. Method Select 98 case of acute exacerbation of asthma in Zhongshan City Henglan hospital during March 2016 to April 2017 as the study group, select the same period the health check-up of 50 volunteers as a control group, the study group was given glucocorticoid impact treatment to ease the change with standard treatment, acute and remission of patients were induced sputum cytological detection of sputum cytology and lung function tests were compared acute asthma attack, remission and healthy people sputum cytology characteristics to explore different treatment options Asthma remission in patients with disease control, analysis of sputum cytology characteristics and pulmonary function. Result Neutrophils in each group were compared, the difference was not statistically significant(P 0.05); eosinophils in the acute phase of asthma, asthma remission, the level of the control group gradually decreased in each group, the difference was statistically significant(P 0.05). Lymphocytes were significantly lower in the remission stage of asthma than those in the acute stage of asthma(P 0.05). The levels of macrophages in the asthma stage were significantly lower than those in the asthma remission stage and the control group(P 0.05) There was no significant difference between the two groups in the control of mild and moderate stable disease(P 0.05). There was no correlation between neutrophils and lung function, eosinophils, lymphocytes and lung function was negatively correlated, macrophages and lung function was positively correlated. Conclusion Sputum cytology in the acute phase of asthma, the steady phase showed a level of change, and pulmonary function have some relevance, standardized treatment can effectively control the patient's condition, induced sputum cytology can determine the severity of asthma, and guide the clinical use of drugs.
