摘要
目的研究2009—2015年山东省潍坊市公立医院住院费用结构变动情况及其变动原因,为医改政策调整提供理论依据。方法选取山东省潍坊市27家公立医院2009—2015年的住院费用数据报表,采用结构变动度分析法进行统计分析,包括结构变动值、结构变动度和结构变动贡献率三个指标。结果 2009—2015年潍坊市年均费用项目中占公立医院住院费用前四位的分别是药品费、治疗费、手术费和卫生材料费。新医改实施以来,公立医院住院费用结构变动较大,总变动度为49.7%,年均变动度为8.28%。卫生材料费、药品费和治疗费是影响住院费用结构变动的主要项目,三者累计贡献率为67.62%。结论新医改背景下的公立医院改革措施,有效降低了住院费用中药占比,住院费用结构趋于合理,但目前住院费用结构仍存在弊端,如医护人员技术价值不能得到体现,卫生材料费用居高不下。
Objective To analyze the structure of inpatient medical expense and the reasons for the changes from2009 to 2015 in Weifang City of Shandong Province in order to provide theoretical evidence for the health care reform.Methods To select the data of 2009 to 2015 hospitalization expenses of 27 public hospitals in Weifang,statistical analysis by structural change degree method,including three indicators of structural change,structural change and contribution rate of structural change.Results In the past seven years,the top four of the public hospital expenses in the annual average cost of Weifang city were medicine,treatment,operation and health materials.Since the implementation of the new medical reform,the structure of hospital expenses in public hospitals has been changed greatly,the total change degree is 49.7%,and the annual average change degree is 8.28%.Health material fee,drug fee and treatment cost are the main items affecting the structural changes of hospitalization expenses,and the cumulative contribution rate of the three is 67.62%.Conclusion The reform of public hospitals under the background of new healthcare reform has effectively reduced the cost of hospitalization,and the cost of hospitalization has tended to be reasonable.However,there are still some drawbacks in the structure of hospitalization expenses,such as the technical value of medical staff can not be reflected,and the cost of health materials is high.
