摘要
目的探讨非确定性人工气道湿化法对脑外伤舌后坠患者的治疗价值。方法选取2016年1月-2017年3月医院收治的脑外伤舌后坠患者116例为研究对象,分为研究组和对照组,每组58例;对照组采用五官科喉头喷雾器间歇湿化法,研究组采用非确定性人工气道(口咽通气道)持续湿化法,记录两组痰液的黏稠程度,通过监护仪记录血氧饱和度(SaO2)、血氧分压(PaO2)、心率(HR)和呼吸频率(RR),至患者出院时,统计两组患者痰痂、气道损伤和肺部感染例数。结果第3天和第5天,研究组痰液的黏稠程度均优于对照组(P<0.05);第3天和第5天,研究组SaO2和PaO2指标均高于对照组,HR和RR指标均低于对照组(P<0.05);通道建立7d内,研究组痰痂、气道损伤和肺部感染发生率均低于对照组(P<0.05)。结论非确定性人工气道持续湿化法湿化效果更好,有利于人体生命体征的稳定,氧合状态更佳,有助于降低肺部感染率,值得临床推荐。
OBJECTIVE To explore the value of non-deterministic artificial airway humidification in treatment of traumatic brain injury patients with glossoptosis.METHODS A total of 116 traumatic brain injury patients with glossoptosis who were treated in the hospital from Jan 2016 to Mar 2017 were recruited as the study objects and divided into the stud group and the control group,with 58 cases in each group.The control group was treated with ENT throat sprayer for intermittent humidification,while the study group was given the non-deterministic artificial airway(oropharyngeal airway)continuous humidification.The viscosity of sputum of the two groups of patients was recorded,the arterial oxygen saturation(SaO_2),partial pressure of arterial carbon dioxide(PaO_2),heart rate(HR)and respiratory rate(RR)were recorded by means of monitor,and the number of cases of sputum scab,airway injury and pulmonary infection was statistically analyzed.RESULTS The viscosity of sputum of the study group was better than that of the control group on Day 3 and Day 5(P〈0.05).The levels of SaO_2 and PaO_2 of the study group were higher than those of the control group on Day 3 and Day 5,and the HR and RR of the study group were lower than those of the control group(P〈0.05).The incidence rates of sputum scab,airway injury and pulmonary infection were lower in the study group than in the control group within 7 days after establishment of airway(P〈0.05).CONCLUSION The non-deterministic artificial airway continuous humidification method shows more remarkable effect on humidification,facilitates the stability of vital signs,achieves better oxygenation state and reduces the incidence rate of pulmonary infection,and it is worthy to be promoted.
