摘要
目的分析骨髓腔穿刺输液技术在院前心肺复苏中的价值。方法据实验设定的纳入标准,采用队列研究的方法,2017年1月—2017年12月河南省人民医院急诊抢救间实施心肺复苏急救的患者,分为实验组和对照组。实验组采用骨髓腔穿刺技术补液,对照组采用外周静脉输液通道补液。比较两组通道建立的时间、首次给药时间、补液速度、ROSC成功率、乳酸清除率等指标。结果实验组建立通道时间小于对照组,实验组首次给药时间小于对照组,实验组补液速度快于对照组(P<0.05);实验组ROSC时间短于对照组,实验组24 h复苏成功率高于对照组,实验组7 d复苏成功率高于对照组(P<0.05);实验组6 h乳酸清除率、24 h乳酸清除率高于对照组(P<0.05)。结论骨髓腔穿刺输液较外周静脉输液更加快速、便捷、有效,能够提高院前心肺复苏患者早期乳酸清除率,提高院前急救心肺复苏成功率,值得推广使用。
Objective To analyze the value of intraosseous infusion technique in pre - hospital cardiopulmonary resuscitation. Methods From January 2017 to December 2017, the patients with CPR in the emergency room of our hospital were divided into experimental group and control group according to the inclusion criteria of the experiment, and the method of cohort study was carried out. The experimental group used intraosseous infusion and the control group used pe- ripheral venous infusion channel. Compared the time between the two sets of channels, the first time for drug administration, the rate of replenishment, the success rate of ROSC, the rate of lactate clearance, etc. Results The experimental group established the channel time less than that of the control group, and the experimental group first gave the drug time less than the control group, and the experimental group was faster than the control group （P 〈 0. 05 ）. In the experimen- tal group, ROSC was shorter than the control group, and the success rate of the experimental group was higher than that in the control group （P 〈 0. 05 ）. The lactate clearance rate of 6 hours and 24 hours in the experimental group was higher than that in the control group （P 〈 0. 05）. Conclusion Intravenous infusion is more rapid and convenient than periph- eral vein infusion, which can improve the early lactate clearance rate in patients with cardiopulmonary resuscitation in the hospital and improve the success rate of CPR before the hospital. It is worth popularizing.
