摘要
目的:探讨在急性中度重症和重症急性胰腺炎治疗中使用凉膈散中西医结合治疗的价值。方法:选择中度重症和重症急性胰腺炎63例,随机将其分为治疗组32例和对照组31例,治疗组在西医综合治疗的基础上加用凉膈散汤剂口服以及灌肠,对照组给予西医综合治疗。记录患者腹痛缓解时间、开放肠内营养时间,1周后复查患者腹部CT影像学的变化,比较2组患者治疗后第1、3、7、10 d血淀粉酶、白细胞总数、C反应蛋白水平,比较2组患者治疗1周后腹内高压、急性呼吸窘迫综合征(ARDS)、急性肾损伤(AKI)并发症的发生率及入住ICU天数。结果:治疗组在临床综合疗效、实验室指标、并发症等方面均优于对照组(均P<0.05)。结论:凉膈散联合西医综合治疗对ICU中度重症和重症急性胰腺炎的临床疗效满意,可缩短患者病程及治疗费用,减少并发症的发生。
Objectives： To investigate the efficacy of Liangge San combined with western medicine in the treatment of moderate and severe acute pancreatitis. Methods： A total of 64 patients with moderate and severe acute pancreatitis were randomly assigned into two groups： 32 cases in treatment group and 31 cases in control group. The treatment group received the same level of western medical therapy as the control group plus oral administration and coloclysis of Liangge San. The changes of serum amylase levels,white blood cell count and C reactive protein were compared at 1st,3rd,7th and 10th day after treatment between two groups. The relief of abdominal pain,the application of enteral nutrition,and the changes of CT abdominal images after treatment for 7 days were recorded. The incidence of intra-abdominal hypertension,ARDS,AKI and length of ICU stay after one week of treatment were noted and compared between two groups. Results： The treatment group was superior to control group in terms of laboratory test results,therapeutic effects and incidence of complications with significant differences（P〈0. 05）. Conclusions： The results indicate that the application of Liangge San with western medical treatment is of positive effect,which can shorten the disease duration,reduce medical cost,and decrease the incidence of complications.
