摘要
目的了解2009-2015年甘肃省各级医疗机构公共卫生资源配置及服务能力现状。方法回顾性调查甘肃省5家省级,7家市(州)级,14家县级医院及27家乡(镇)卫生院公共卫生科建设、人员和硬件配置、组织管理、公共卫生服务能力等情况,访谈公共卫生科责任人。结果 48家医疗机构设置了公共卫生科,5家虽未单独建立,但隶属于护理部、中医科或预防接种室,办公总面积3 257m2,配备专用电脑158台,工作人员436人次,年龄中位数32.90岁,专职人员279人次,大专及以上学历76.61%,预防医学专业9.17%,省、市(州)级人员学历和预防专业(公共卫生)均高于县级及乡(镇)卫生院(χ2=17.421,P=0.000;χ2=24.497,P=0.000),非在岗人员63.16%调至其他科室。100.00%医疗机构开展医院感染预防控制、传染病诊疗管理、预防接种和慢病管理工作,14.81%和22.22%开展职业病管理和放射安全防护。结论甘肃省医疗机构公共卫生科建设、硬件配备和日常运营状况良好,专业人员素质有待加强,公共卫生服务范围有待明确和规范,服务能力有待提高。
Objective To better understand the overall status of public health departments and service capability in medical institutions in Gansu from 2009 to 2015. Methods A retrospective survey was conducted in public health departments of total 54 medical institutions,including 5 of provincial level,7 of city level,14 of county level and 27 of township level in terms of department construction,management,personnel composition,hardware allocation,duty execution and service capability,the directors of the departments were also interviewed. Results Every medical institution surveyed had public health departmen with overall office areas of 3 257 m^2,158 dedicated computers and 436 staff.The median age was 32.90,279 were specialized staff,96.61% owned associate degree and above and 9.17% graduated with preventive medicine major.Personnel in provincial level and city level medical institutions usually had higher degree compared with those in county level and township level ones and more personnel had major of preventive medicine（χ^2=17.421,P=0.000;χ^2=24.497,P=0.000）.100% of medical institutions were capable of doing jobs as nosocomial infection prevention control,management for infectious disease diagnosis and treatment,prophylactic vaccination and chronic disease management,only 14.81% and 22.22% hospitals could work on occupational disease management and radiological-safety protection. Conclusions The departments of public health in medical institutions of Gansu operate well which are equipped with reasonable hardware and relatively high-quality personnel.However the service capability needs to be further improved.
