摘要
目的:观察电针针刺法结合吞咽训练对重度颅脑损伤有意识障碍患者吞咽障碍的临床疗效。方法:对40例重度颅脑损伤具有意识障碍的吞咽功能障碍患者随机分为治疗组和对照组各20例,治疗组采用电针针刺结合吞咽功能训练治疗,对照组在改善脑功能药物基本上给予吞咽功能训练。治疗每天1次,每周6次,4周为1疗程,共治疗3个疗程。采用洼田饮水试验评价吞咽功能,并用日本康复学界的吞咽困难评价标准评价疗效。结果:治疗组吞咽功能改善有效率为80.0%,优于对照组的的65.0%(P<0.05)。治疗3个疗程后两组均改善吞咽困难功能评分及洼田饮水试验分级(均P<0.05),治疗组优于对照组(均P<0.05),且与对照组比较差异有统计学意义(P<0.05)。结论:重度颅脑损伤意识障碍患者同样可进行吞咽训练,电针针刺疗法结合吞咽训练能有效地改善其吞咽功能,提高生存预后。
Objective： To observe the clinical effect of electro-acupuncture combined with swallowing training on dys- phagia in patients with disturbance of consciousness after severe eranioeerebral injury. Methods： 40 patients with severe craniocerebral trauma with swallowing dysfunction who had disturbance of consciousness were randomly divided into treatment group and control group with 20 patients each. The treatment group was treated with electro-acupuncture com- bined with swallowing function training. The control group was basically gave swallowing function training and improving brain function drugs. The treatment was once a day, 6 times a week, and 4 weeks was a course of treatment. There were a total of 3 courses of treatment. The evaluation of swallowing function was conducted using the Putian drinking water test, and the therapeutic effect was evaluated using the dysphagia evaluation criteria of the Japanese rehabilitation community. Results： The effective rate of swallowing function in the treatment group was 80.0%, which was better than 65.0% in the control group （P〈0.05）. After the three courses of treatment, both groups improved the score of dysphagia and the grade of Putian drinking water test （all P〈0.05）. The treatment group was better than the control group （all P〈0.05）, and there was a statistically significant difference compared with the control group （P〈0.05）. Conclusion： Patients with severe brain in- jury with consciousness impairment can also perform swallowing training. Electro-acupuncture combined with swallowing training can effectively improve swallowing function and improve survival prognosis.
