摘要
目的:探讨精神分裂症前驱期患者的中医证素特点。方法:根据中医证素标准,对符合精神分裂症前驱期诊断标准的105例精神分裂症前驱期患者研究其中医证候特点、中医证素的分布规律、证素与精神分裂症前驱期量表评分的相关性,以确定该病的中医病机。结果:精神分裂症前驱期常见症状为注意散漫、舌质红、左脉弦、妄想等,主要病位要素为肾与肝,主要病性要素为阴虚与火热。4种证素之间相关性显著(P<0.01),4种证素都和SIPS总分及阳性症状总分相关性显著(P<0.01),肝、肾证素与阴性症状总分相关性显著(P<0.01,P<0.05),肾、火热和阴虚与瓦解性症状总分相关性显著(P<0.05,P<0.01)。结论:精神分裂症前驱期的主要病位要素为肾与肝,主要病性要素为阴虚与火热,核心病机可能为"肾虚肝旺"。
Objective： To investigate the characteristics of syndrome elements of prodromal schizophrenia patients. Methods： According to associated standard in traditional and western medicine, the TCM symptoms of 105 patients with prodromal schizophrenia were investigated and performed statistical analysis. Results： Lack of attention, red tongue, string pulse on left, delusion, etc. were common symptoms of TCM; Kidney and Liver were main syndrome elements of disease location, syndrome element of disease character was yin deficiency and fire. There were significant correlations among the four syndrome elements （P〈0.01）. The relevance of four syndrome elements and total score of SIPS and positive symptoms was significant （P〈0.01）. Liver and kidney had significant correlations between the total scores of negative symptoms （P〈0.01, P〈0.05）. Kidney, fire and yin deficiency had significant correlations between the total scores of disintegrating symptoms （P〈0.05, P〈0.01）. Conclusion： In prodromal schizophrenia, kidney and liver are main syndrome elements of disease location; syndrome element of disease character is yin deficiency and fire. The syndrome of ＇kidney deficiency and liver hyperactivity＇ may be the core pathogenesis of prodromal schizophrenia.
