摘要
目的探讨超声联合磁共振诊断胎儿中枢神经系统畸形的应用价值。方法选取2014-06—2016-06大连大学附属中山医院收治的经超声筛查怀疑胎儿中枢神经系统畸形的孕妇110例为研究对象,行超声联合磁共振检查,以胎儿出生后检查或胎儿尸检结果为评定金标准,观察联合检查对胎儿中枢神经系统畸形诊断的应用价值。结果超声联合磁共振检查出胎儿中枢神经系统畸形79例86处,其灵敏度、特异度、阳性预测值、阴性预测值和准确度与单纯磁共振检查结果相近,除特异度外,其余各指标检测值均高于单纯超声检查结果。结论超声联合磁共振可获得最佳影像优势,对胎儿中枢神经系统畸形具有较好的诊断价值。磁共振作为补充手段,可弥补单纯应用超声带来的不确定性,减少漏诊率。
Objective To investigate the value of ultrasound combined with magnetic resonance imaging in the diagnosis of fetal central nervous system malformation. Methods 110 pregnant women who suspected the fetal central nervous system malfor mation by ultrasound in our hospital from June 2014 to June 2016 were selected. The value of combined examination on the diagnosis of fetal central nervous system malformation was observed. Results The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of the 110 cases of fetal central nervous system malformation detected by ultrasound combined with magnetic resonance imaging were similar. The other indexes were significantly higher than those examined by ultrasound only. Conclusion Ultrasound combined with magnetic resonance imaging can obtain the best imaging advantages, and has a good diagnostic value for fetal central nervous system malformation. But the ultrasonic examination is simple to operate with low cost, and is still the main screening means of fetal central nervous system malformation; magnetic resonance imaging acts as a complementary method,can make up for the simple application of ultrasound uncertainty and reduce the rate of missed diagnosis.
