T2 mapping在膝关节退变伴局部骨坏死灶与骨髓水肿的影像学探讨 被引量:1

摘要 目的:探讨T2 mapping技术对膝关节退行性改变伴局部骨坏死与骨髓水肿的影像学表现。方法:30例患者行膝关节MRI检查,行常规MRI扫描及T2 mapping成像。根据关节骨坏死灶在T2mapping上色阶分布情况进行分组,描绘感兴趣区并测量其平均T2值,比较三组T2值差异并分析原因。结果:膝关节面下骨质坏死灶在常规MRI扫描呈结节或片状长T1长T2影,其内信号略欠均匀,内部结构信号未见明显分界。T2mapping上可见病灶骨近关节面为低色阶信号,其两侧及远离关节面骨质为高色阶二种相反信号影,边界清晰。进行勾边取感兴趣区分别测量平均T2值,并与正常骨质T2值进行比较,三组T2值统计结果 p<0.001,有明显统计学差异,对低色阶区病灶行纵向测量,将病灶骨关节面起始部向远离关节面方向纵向平均分成三组,分别测量每组平均T2值,其三组T2值统计结果 p<0.001,差异有明显显著性。。结论:本研究可以明确区分膝关节退行性改变伴局部骨质脂肪坏死崩解伴纤维肉芽组织与周围骨髓水肿的界限,为临床提供精准的影像表现。 Objective:To investigate the effect of T2 mapping on the knee joint degenerative changes with local bone necrosis and bone marrow edema imaging.Methods:30 cases of patients with knee joint MRI examination,routine MRI scan and T2 mapping imaging.According to the joint bone necrosis in the T2 mapping levels and distribution in groups,depicting the region of interest and measurement of the average value of T2,the differences and analyze the reasons for value of T2 among the three groups.Results:the articular surface of bone under necrotic nodule or flake long T1 T2 in conventional MRI scanning,the signal is slightly less uniform,the internal structure of no signal.T2 mapping visible lesions were found near the articular surface of bone for low signal levels,the two sides and away from the articular surface of bone for high levels of two opposite signal and clear boundary.Crochet regions of interest were measured respectively the average T2 value,and compared with normal bone T2 values,T2 values of three groups statistical results P〈0.001.There was significant difference on low levels for longitudinal measurements of lesions,lesions of bone and joint surface away from the articular surface of the initial part of the longitudinal direction were divided into three groups,each group were measured and the average T2 value,T2 value of the three groups of P〈0.001 statistics,the difference was significant.Conclusion:This study can clearly distinguish the degenerative changes of the knee joint with local fatty necrosis of bone,disintegration with fibrous granulation tissue and peripheral bone marrow edema,and provide accurate imaging findings for the clinical.
