平均血小板体积与动脉型肺动脉高压关系的研究 被引量:2

摘要 目的探讨平均血小板体积(MPV)与动脉型肺动脉高压(PAH)的关系及在PAH中的预测价值。方法选择2008年7月至2016年8月在沈阳军区总医院住院的PAH患者328例,其中男性93例,女性234例,平均年龄32.4岁。选择同时间段在体检中心体检的178例健康人群作为对照组,其中男性42例,女性136例;平均年龄34.3岁。两组均进行血液分析检查、生物化学检查、心电图及超声心动图检查,其中PAH患者均行右心导管检查。结果 PAH组MPV较对照组高[(9.08±1.20)fL vs(8.15±0.74)fL,P<0.001]。在PAH组中,其中轻中度44例,重度284例。重度PAH MPV较轻中度PHA高[(9.2±1.1)fL vs(8.6±1.3)fL;P<0.05]。受试者工作特征曲线(ROC)分析,MPV>8.75 fL作为分界点预测PAH的灵敏度为83%,特异度为59%。结论 MPV水平与PAH存在明显相关性,MPV能作为诊断PAH的一种炎性标记物。 Objective To investigate relationship between mean platelet volume(MPV) levels and pulmonary arterial hypertension(PAH), and study predictive value in PAH. Methods From July 2008 to August 2016, 328 PAH patients(PAH group)were enrolled, which included 93 males and 234 females, aged 10-69 years old with mean age of 32.4 years old. At the same time, 178 healthy were set as control group, which included 42 males and 136 females, aged 11-68 years old with mean age of 34.3 years old. The blood analysis, biochemistry, electrocardiogram(ECG) and echocardiography examination were tested in all of them, and the right heart catheterization examination was performed in PAH patients. Results The MPV levels were significantly higher in PAH than that in control group[(9.08 ± 1.20) fL vs(8.15 ± 0.74) fL; P〈0.001]. In PAH group, 44 cases were light-middle PAH and 284 were severe. The MPV level of severe PAH was higher than light-middle PAH[(9.2 ± 1.1) fL vs(8.6 ± 1.3) fL; P〈0.05]. In receiver operating characteristics curve analysis, the cut-off value of MPV levels was 8.75 fL,while 83 % sensitivity and 59 % specificity in predicting PAH. Conclusion It is demonstrated that there is a significant relationship between MPV level and PAH, the MPV level could be an inflammatory marker to diagnose in PAH.
