摘要
目的观察独活寄生汤联合塞来西布胶囊防治早期膝骨性关节炎的临床疗效。方法筛选76例早期膝骨性关节炎患者,采用计算机随机数字表法分为联合组和西药组,联合组38例患者采用中西医结合疗法,即同时给予口服独活寄生汤和塞来昔布治疗,对照组38例患者仅给予塞来昔布胶囊口服治疗。观察两组患者治疗前后临床表现,检测患者膝骨性关节液中基质金属蛋白酶(MMP-3)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-1β(IL-1β)水平,评价其临床效果。结果联合组治疗后总有效率明显优于西药组(P<0.01)。两组患者膝关节液中MMP-3、TNF-α、IL-6、IL-1β含量治疗后较本组治疗前明显降低(P<0.05),且联合治疗组较西药组治疗后降低幅度更明显(P<0.05)。结论独活寄生汤联合塞来昔布胶囊防治早期膝骨性关节炎疗效较单用西药疗效好,但其作用机制仍需进一步深入研究。
Objective To observe the clinical therapeutic effects on early knee osteoarthritis(KOA)treated with duhuo jisheng decoction and celecoxib capsules. Methods A total of 76 patients of early KOA were screened and divided into an allied therapy group and a western medicine group according to the random number table. In the allied therapy group(38 cases),the integrated Chinese and western medicine was used with the oral administration of duhuo jisheng decoction and celecoxib capsules. In the western medicine group(38 cases),celecoxib capsules were only prescribed for oral administration. The clinical manifestations were observed before and after treatment in the two groups,as well as the levels of MMP-3,TNF-α,IL-6 and IL-1β in the synovial fluids of knee. The clinical effects were evaluated. Results The total effective rate in the allied therapy group was better remarkably than the western medicine group(P〈0. 01). The levels of MMP-3,TNF-α,IL-6 and IL-1β in the synovial fluids of knee after treatment were reduced remarkably as compared with those before treatment in the two groups(P〈0. 05). The reducing degree in the allied therapy group was more remarkable than the western medicine group(P〈0. 05). Conclusion The allied treatment with duhuo jisheng decoction and celecoxib capsules achieves the better effects on early KOA as compared with the simple use of western medicine. The effect mechanism needs to be further researched.
