摘要
目的探讨参麦注射液与无创机械通气联合持续静脉输注吗啡对急性左心功能衰竭患者动脉血气指标及BNP、NT—proBNP水平的影响。方法选取本院2014年6月至2017年2月收治的66例急性左心功能衰竭患者,根据治疗方法不同分组,各33例。对照组予以无创机械通气+持续静脉输注吗啡治疗,观察组予以参麦注射液+无创机械通气+持续静脉输注吗啡治疗。对比两组疗效及不良反应发生率,检测两组治疗前后血浆BNP、NT—proBNP水平以及血气指标(pH、PaO2、PaCO2、SaO2)变化情况。结果观察组治疗总有效率为93.94%(31/33),对照组为72.73%(24/33),差异有统计学意义(P〈0.05);治疗前两组患者BNP、NT—proBN水平对比,差异均无统计学意义(均P〉0.05),治疗后观察组患者BNP、NT—proBN水平均低于对照组,差异均有统计学意义(均P〈0.05);治疗前两组患者pH、PaO2、PaCO2、SaO2比较,差异均无统计学意义(均P〉0.05),治疗后观察组患者pH、PaO2、SaO2高于对照组,PaCO2低于对照组,差异均有统计学意义(均P〈0.05)。结论参麦注射液与无创机械通气联合持续静脉输注吗啡治疗急性左心功能衰竭,效果显著,可改善患者动脉血气指标,降低BNP、NT—proBNP水平,促进病情良好转归。
Objective To investigate the influence of Shenmai injection and noninvasive mechanical ventilation combining continuous intravenous infusion of morphine on arterial blood gas index and levels of BNP and NT-proBNP in patients with acute left heart failure. Methods 66 patients with acute left ventricular failure treated at our hospital from June, 2014 to February, 2017 were selected and divided into a control group and an observation group according to different treatment methods, 33 cases in each. The control group were treated with noninvasive mechanical ventilation and continuous intravenous infusion of morphine and the observation group with Shenmai injection, non-invasive mechanical ventilation, and continuous intravenous infusion of morphine. The efficacy and incidence of adverse reactions were compared between the two groups. The levels of plasma BNP and NT-proBNP and blood gas indexes (pH, PaO2, PaCO2 and SaO2) were measured before and after the treatment in the two groups. Results The total effective rate was higher in the observation group than in the control group [93.94% (31/33) vs. 72.73% (24/33)], with a statistical difference (P 〈 0.05). Before the treatment, there were no statistical differences in the levels of BNP and NT-proBN between the two groups (P 〉 0.05). After the treatment, the levels of BNP and NT-proBN were lower in the observation group than in the control group, with statistical differences (P 〈 0.05). There were no statistical differences in pH, PaO~, PaCOz, and SaO2 between the two groups before the treatment ( P 〉 0.05 ) . After the treatment, the pH, PaO2, and SaO2 were higher and PaCO2 was lower in the observation group than in the control group, with statistical differences (P 〈 0.05). Conclusion Shenmai injection and noninvasive mechanical ventilation combining continuous intravenous infusion of morphine in the treatment of acute left heart failure is effective and can improve the arterial blood gas indexes, reduce the levels of BNP and
