摘要
目的探讨运脾化湿方对小儿轮状病毒性肠炎（pediatric rotavirus enteritis，PRE）湿热证的免疫调节作用。方法收集2014年9月至2016年4月江苏省第二中医院收治的确诊的83例轮状病毒性肠炎患儿，临床症状符合湿热证，随机分为对照组（40例）和观察组（43例）。对照组给予口服补液盐，黏膜保护剂蒙脱石散及微生态制剂枯草杆菌二联活菌治疗，观察组在对照组基础上给予口服运脾化湿方。分别采用流式细胞仪和透射比浊法检测所有患者治疗前后的淋巴细胞亚群和免疫球蛋白水平，并进行组间比较。结果治疗前，两个组别的CD3、CD4T淋巴细胞亚群和CD4／CD8等，IgA、IgG和IgM等免疫球蛋白，大便次数、大便性状、发热和呕吐等主要症状的差异无统计学意义（P〉0．05），治疗后，与对照组相比，观察组的T淋巴细胞亚群明显增高（P〈0．05），免疫球蛋白水平明显增高（P〈0．05），主要症状积分明显降低（P〈0．05）。与对照组比，观察组的无效例数较少，总有效人数较多（P〈0．05）。结论运脾化湿方可用于治疗小儿轮状病毒性肠炎湿热证，其机理可能与改善免疫功能有关。
Objective To investigate the effect of Yunpihuashidecoction in the treatment of pediatric ro- tavirus enteritis（PRE） with damp heat syndrome and its immunoregulatory effect. Methods From September 2014 to April 2016,83 rotavirus enteritis children with damp heat syndrome enrolled in our hospital were divid- ed into the control group （n = 40） and the observation group （n = 43 ）. The control group were given oral rehy- dration salts,mucosal protective agent montmorillonite powder, and microbial preparation Bacillus subtilis. The observation group were given Yunpihuashidecoction based on the standard treatment as in the control group. The lymphocyte subsets and immunoglobulins of all patients were detected by flow cytometry and transmission turbi- dimetric method before and after treatment. Results Before treatment, there was no significant differences in T lymphocyte subsets （ CD3, CIM and CD4/CD8 ）, immunoglobulin （ IgA, IgG and IgM）, stool frequency, stool fea- ture, fever and vomiting （ P 〈 0. 05 ）, between the observation and control groups. After treatment, compared with the control group, the T lymphocyte subsets and immunoglobulin had increased （ P 〈 0. 05 ） , the main symptom score had significantly decreased （P 〈 0. 05 ） in the observation group. Compared with the control group, the in- valid cases in the observation group was less and the total effective cases was more （P 〈 0. 05 ）. Conclusion The Yunpihuashidecoctioncan can treat pediatric rotavirus enteritis with damp heat syndrome by improving im- mune function.
