摘要
目的评价CD64、IP-10及CRP检测在细菌感染性疾病诊断中的意义。方法收集本院儿科2015年1月-2017年1月细菌感染儿童86例,非细菌感染儿童45例及健康儿童35例,采用流式细胞术检测外周血CD64指数、ELISA法和免疫透射比浊法测定血清中的IP-10及CRP的含量。结果细菌感染组的WBC计数明显高于非细菌感染组及对照组(P<0.05),非细菌感染组与对照组比较差异无统计学意义(P>0.05);细菌感染组及非细菌感染组CD64指数、IP-10均明显高于对照组,且细菌感染组明显高于非细菌感染组,差异均有统计学意义(P<0.05);当CD64荧光指数截断值为7.326时,其诊断灵敏度为86.05%,特异度为91.11%,诊断效率均优于单独应用CRP、IP-10;当联合应用CD64指数、IP-10及CRP 3项指标,诊断的灵敏度上升为94.19%,而特异度为88.89%。结论联合CD64指数、IP-10及CRP检测对提高儿童早期细菌感染诊断的敏感性和特异性具有重要价值,有助于儿童早期细菌感染的鉴别诊断。
Objective To evaluate the significance of IP-10,CD64 and C-reactive protein( CRP) detection in the diagnosis of bacterial infectious diseases. Methods A total of 86 newborn infants( bacterial infection group),45 children with non-bacterial infection( non-bacterial infection group) and 35 cases of neonatal( healthy control group) were enrolled in this study. Flow cytometry was used to detect the expression of CD64 in the peripheral blood. The levels of IP-10 and CRP in serum were determined by ELISA and immunofluorescence turbidimetric assay. Results The WBC count of bacterial infection group was significantly higher than that of non-bacterial infection group and healthy control group( P〈0. 05). There was no statistical significance on the difference between non-bacterial infection group and healthy control group( P〉0. 05); CD64 index and IP-10 of the bacterial infection group and non-bacterial infection group were higher than that of the control group( P〈0. 05),and that in the bacterial infection group was significantly higher than the non-bacterial infection group( P〈0. 05).When the cutoff value of CD64 fluorescence index was 7. 326,the diagnostic sensitivity was 86. 05% and the specificity was91. 11%,which was better than that of CRP or IP-10 alone. When the CD64 index,IP-10 and CRP were used in combination,the sensitivity was 94. 19%,and the specificity was 88. 89%. Conclusion The combination of CD64 index,IP-10 and CRP detection is of great value to improve the sensitivity and specificity of early neonatal bacterial infection,which is helpful for the differential diagnosis of early bacterial infection in neonates.
