摘要
目的探讨腹腔镜联合亮丙瑞林治疗子宫内膜异位症伴不孕的效果情况。方法分析我院2014年2月~2017年2月收治的子宫内膜异位症伴不孕患者80例临床资料,依据治疗方式不同进行分组,对照组(单独腹腔镜手术治疗)40例和观察组(腹腔镜手术联合亮丙瑞林治疗)40例。观察两组患者术中出血量、手术时间、术后肛门排气时间、术后下床活动时间、手术前后促卵泡生成素(FSH)、雌二醇(E_2)、黄体生成素(LH)及更年期指数(KMI)评分、临床疗效情况。结果观察组患者术中出血量、手术时间、术后肛门排气时间、术后下床活动时间均少于对照组,差异均有统计学意义(P<0.05)。两组患者术前FSH、E_2、LH及KMI评分差异无统计学意义(P>0.05),术后两组患者FSH、E_2、LH均低于术前,KMI评分高于术前,观察组患者术后FSH、E_2、LH均低于对照组,KMI评分高于对照组,观察组患者临床治疗总有效率高于对照组,差异有统计学意义(P<0.05)。结论腹腔镜联合亮丙瑞林治疗子宫内膜异位症伴不孕患者,具有创伤小,恢复快,降低性激素水平,提高临床疗效,值得临床推广应用。
Objective To investigate the effect of laparoscopy combined with Leuprorelin in the treatment of en- dometriosis complicated with infertility.Methods From February 2014 to February 2017,clinical data from 80 endometriosis patients with infertility in our department were retrospectively analyzed.According to different treatment methods,they were evenly divided into control group （n=40,1aparoscopic surgery alone） and observation group （n=40,la- paroscopic surgery combined with Leuprorelin）.The intraoperative bleeding volume,operation time,postoperative anal ex- haust time,postoperative ambulation time,the levels of follicle stimulating hormone （FSH）,estradiol （E2）,luteinizing hor- mone （LH）,and Kupperman menopause index （KMI） score before and after the operation,and clinical efficacy were ob- served in the two groups.Results The intraoperative bleeding volume,operation time,postoperative anal exhaust time, and postoperative ambulation time in the observation group were all lower than those in the control group,with statistical significance （P〈0.05）.There was no great difference in FSH,E2,LH,or KMI score before surgery in both groups （P〉0.05）. After surgery,the levels of FSH,E2,and LH in the two groups were all lower compared with those before surgery.The KMI score was higher.Meanwhile,the levels of FSH,E2,and LH were lower and KMI score was higher in the observation group than in the control group after surgery.The total clinical therapeutic effectiveness rate in the observation group was higher in comparison with that in the control group,with a statistical difference （P〈0.05）,Conclusion Combination of laparoscopy and Leuprorelin in the treatment of endometriosis associated infertility patients has advantages of small trauma and quick recovery,reduce the levels of sex hormones,and improve clinical efficacy,which is worthy of clinical application and promotion.
