摘要
目的探讨不同断脐时间对经阴道分娩早产儿的影响。方法选择2016年于西北妇女儿童医院阴道分娩的早产孕妇394例,随机分为观察组和对照组,观察组在分娩后30~120 s断脐,对照组在分娩后10~30 s断脐,观察两组新生儿红细胞压积、血红蛋白、贫血率、输血率、经皮胆红素(transcutaneous bilirubin,TCB)峰值、光疗天数、高胆红素血症、颅内出血情况。结果与对照组比较,观察组新生儿产后1、3、7、14和28 d时红细胞压积、血红蛋白和红细胞数目均显著增高;贫血率和输血率均显著降低[(14.21%vs 22.34%)和(3.55%vs 8.12%)],差异均有统计学意义(P<0.05)。两组新生儿TCB峰值、光疗天数、高胆红素血症和颅内出血率比较,差异无统计学意义(P>0.05)。结论胎儿娩出后,脐动脉搏动减弱或消失时再断脐可能有助于提高新生儿红细胞压积、血红蛋白和红细胞数目,降低贫血发生率。
Objective To explore the influence of different time of cord clamping on preterm infants of vaginal birth. Methods A total of 394 preterm pregnant women who delivered vaginally at Northwestern Women and Children's Hospital from January 2016 to December 2016 were enrolled and divided randomly into observation group and control group. The infants in the observation group were received omphalotomy between 30 and 120 seconds after the delivery while the ones in the control group were received omphalotomy within 30 seconds after the delivery. The hematocrit, the hemoglobin, red blood cell count, the anemia rate, the blood transfusion rate, the peak value of transcutaneous bilirubin (TCB), phototherapy days, hyperbilirubinemia, and the intracranial hemorrhage of both groups were observed. Results Compared with the control group, the hematocrit, hemoglobin and red blood cell count of the observation group' s infants on 1, 3, 7, 14 and 28 days after the delivery increased significantly ( P 〈 0. 05 ) ; while the anemia rate and the blood transfusion rate decreased apparently [ (14. 21% vs. 22. 34% ) and (3. 55% vs. 8.12% ) ]. There were no statistical differences in TCB peak value, phototherapy days, hyperbilirubinemia and the intracranial hemorrhage between the two groups ( P 〉 0. 05). Conclusion After the baby is delivered, cord clamping at the timing of umbilical artery pulsatility weakened or disappeared may help to increase the number of hematocrit, hemoglobin and red blood cells, and reduce the incidence of anemia.
