摘要
目的系统评价我省增补基本药物川芎嗪治疗过敏性紫癜的疗效和安全性,为临床及今后我省增补目录的遴选提供循证参考。方法计算机检索Pub Med、中国期刊网全文数据库(CNKI)、万方医学网、维普中文科技期刊全文数据库(VIP)数据库,收集川芎嗪治疗过敏性紫癜的随机对照试验(RCT),提取资料并评价质量后,采用Rev Man 5.3统计软件进行Meta分析。结果共纳入16项RCT,合计1290例患者。Meta分析结果显示,试验组在总有效率[OR=5.64,95%CI(2.34,13.62),P<0.0001]、预防出现肾损害比例[OR=0.23,95%CI(0.13,0.39),P<0.00001]、复发率[OR=0.36,95%CI(0.19,0.67),P<0.001];紫癜消退时间[MD=-3.16,95%CI(-5.28,-1.05),P=0.003]、关节症状缓解时间[MD=-1.89,95%CI(-2.34,-1.44),P<0.00001]、胃肠道症状缓解时间[MD=-2.55,95%CI(4.18,-0.91),P=0.002]等方面均优于对照组,差异均有统计学意义。9个研究报道试验组未发生不良反应。结论川芎嗪治疗过敏性紫癜有较好疗效且不良反应发生较少。
OBJECTIVE To systematically review the efficacy of Ligustrazine in the treatment of HenochSchonlein Purpur,and provide evidence-based reference for clinical treatment and for the selection of additional essential medicine list. METHODS Retrieved from Pub Med,CNKI,Wanfang and VIP database,randomized controlled trials（ RCT） about the efficacy of Ligustrazine in the treatment of Henoch-Schonlein Purpur were collected. Meta-analysis was performed by using Rev Man 5. 3 software after data extraction and quality evaluation. RESULTS Totally 16 RCTs were enrolled,involving 1290 patients. Results of Meta-analysis showed,overall efficiency[OR = 5. 64,95% CI（ 2. 34,13. 62）,P〈0. 0001 ],kidney involvement of HSP[OR = 0. 23,95% CI（0. 13,0. 39）,P〈0. 00001],recurrence rate[OR = 0. 36,95% CI（0. 19,0. 67）,P〈0. 001],purpura subsided time[MD =-3. 16,95% CI（-5. 28,-1. 05）,P = 0. 003],joint symptoms disappeared time[MD =-1. 89,95%CI（-2. 34,-1. 44）,P〈0. 00001 ]and gastrointestinal symptoms disappeared time[MD =-2. 55,95% CI（-4. 18,-0. 91）,P = 0. 002 ]in test group were significantly better than control group,there were significant differences between the 2 groups. 9 trials reported no adverse reactions in the experimental group. CONCLUSION Ligustrazine in the treatment of Henoch-Schonlein Purpur has good curative effect and adverse reaction has less occurred.
