摘要
目的探究经皮锁定加压钢板固定与交锁髓内钉固定治疗胫骨远端关节外骨折的临床效果。方法选取我院2013年3月至2017年3月收治的100例胫骨远端关节外骨折患者,按固定方法不同分为观察组和对照组,各50例。分别给予对照组交锁髓内钉固定,观察组患者进行经皮锁定加压钢板固定,对两组患者的临床指标、Mazur评价分级与并发症情况进行比较。结果观察组骨折愈合时间长于对照组(P<0.05),而手术时间、出血量、住院时间、Mazur评价分级情况比较,差异不显著(P>0.05);手术后,观察组畸形愈合率(4.00%)低于对照组(20.00%)(P<0.05);两组感染情况、延迟愈合情况、骨折不愈合情况比较,差异均不显著(P>0.05)。结论经皮锁定加压钢板固定和交锁髓内钉固定治疗胫骨远端骨折均具有较好的效果,但交锁髓内钉固定治疗后患者骨折愈合速度较快,而经皮锁定加压钢板固定的畸形愈合情况较少。
bObjective To investigate the clinical effects of percutaneous locking compression plate and interlocking intramedullary nailing in the treatment of distal tibial fracture. Methods One hundred cases of extra articular distal tibial fractures patients in our hospital from March 2013 to March 2017 were divided into observation group and control group with different fixation methods, with 50 cases in each group. The control group was treated with interlocking intramedullary nail fixation, and the observation group was treated with percutaneous locking compression plate fixation. The clinical pa-rameters, Mazur scores and clinical complications of the two groups were compared. Results The fracture healing time of the observation group was longer than that the of the control group(P〈0.05). The operation time, bleeding volume, hospitalization time and Mazur scores had no statistical difference between the two groups(P〈0.05); after operation, the deformity healing rate of the observation group was 4.00%, it was lower than that of the control group(20.00%)(P〈0.05). There were no significant differences in the rates of infection, delayed union and nonunion of the fracture between the two groups(P〈0.05). Conclusion Percutaneous locking compression plate fixation and interlocking intramedullary nail fixation in the treatment of distal tibial fractures has good effect, but the interlocking intramedullary nail fixation in the treatment of pa-tients with fracture healing faster, and percutaneous locking compression plate fixation malunion less.
