摘要
目的观察负压封闭引流技术治疗高位肛周脓肿的临床效果。方法将我院收治的68例高位肛周脓肿患者随机分为对照组和观察组,各34例,分别采用传统切开挂线术与负压封闭引流术治疗。比较两组的治疗效果、治疗前后疼痛、肛门失禁改善情况,比较两组治疗前后血清TNF-α、EGF含量。结果观察组治疗后4周,总有效率显著高于对照组(P<0.05)。治疗后,两组VAS及Wexner评分、血清TNF-α、EGF含量均改善(P<0.05)。观察组VAS、Wexner评分显著低于对照组,TNF-α含量显著低于对照组,EGF含量高于对照组(P<0.05)。结论采用负压封闭引流治疗高位肛周脓肿,临床效果显著,可显著缓解疼痛,改善肛门失禁,这可能与抑制TNF-α水平,上调EGF水平有关。
Objective To observe the clinical effect of negative pressure sealing drainage in the treatment of high perianal abscess. Methods Sixty-eight patients with high perianal abscess admitted in our hoapital were randomly divided into control group and observation group, with 34 cases in each group, the incision-thread-drawing procedure and vacuum sealing drainage were used respectively. The therapeutic effects, pain and anus incontinence were compared between the two groups before and after treatment, and the levels of TNF-α and EGF in the two groups were compared before and after treatment.Results Four weeks after treatment, the total effective rate in the observation group was significantly higher than that in the control group（P〈0.05）. After treatment, the VAS, Wexner scores, levels of serum TNF-α and EGF significantly improved in both groups（P〈0.05）. The VAS and Wexner scores in the observation group were significantly lower than those in the control group, the content of TNF-α in the observation group was significantly lower than that in the control group, and the EGF content was higher than that in the control group（P〈0.05）. Conclusion The use of vacuum sailing drainage in the treatment of high perianal abscess has exact clinical effect, and it can significantly relieve pain, improve anal incontinence, which may be related to inhibition of TNF-α levels, up-regulation of EGF levels.
