摘要
目的探讨初诊2型糖尿病(T2MD)患者血糖波动与甲状腺激素水平的相关性。方法选取2014年1月至2017年1月北京和睦家医院收治的初诊T2MD患者210例作为T2MD组,选取同期糖调节正常者95例作为对照组,采用动态血糖监测系统(CGMS)对两组患者进行连续72 h血糖监测,并计算日内平均血糖波动幅度(MAGE)、日间血糖平均绝对差(MODD)、最大血糖波动幅度(LAGE),同时测定糖化血红蛋白(Hb A1c)、四碘甲腺原氨酸(T4)、三碘甲腺原氨酸(T3)、反三碘甲状腺原氨酸(r T3)、游离四碘甲腺原氨酸(FT4)、游离三碘甲腺原氨酸(FT3)、促甲状腺激素(TSH)变化;根据Hb A1c水平分为<6.5%组和≥6.5%组,比较甲状腺激素变化和血糖波动情况,并分析两者相关性。结果 T2MD患者MAGE、MODD、LAGE、Hb A1c、r T3水平明显高于对照组,T3、FT3水平明显低于对照组,差异有统计学意义(P<0.05);Hb A1c<6.5%组MAGE、MODD、LAGE、r T3水平低于Hb A1c≥6.5%组,T3、FT3水平明显高于Hb A1c≥6.5%组,差异有统计学意义(P<0.05);Pearman相关分析显示,T3、FT3与MAGE、MODD、LAGE、Hb A1c呈负相关关系(P<0.05),r T3与MAGE、MODD、LAGE、Hb A1c呈正相关关系(P<0.05)。结论初诊T2MD患者存在T3、FT3水平下降,r T3水平上升,且与血糖波动情况有关。
Objective To investigate the correlation between glucose fluctuation and levels of thyroid hor-mones in patients with newly diagnosed type 2 diabetes mellitus (T2MD). Methods A total of 210 patients with newlydiagnosed T2MD treated in Beijing United Family Hospital from January 2014 to January 2017 were selected and includ-ed in the T2MD group, and 95 subjects with normal glycoregulation were selected as the control group at the same time.The continuous glucose monitoring system (CGMS) was used for 72 h continuous blood glucose monitoring in the twogroups. The mean amplitude of glycemic excursions (MAGE), the mean of daily differences (MODD) and the largest am-plitude of glycemic excursions (LAGE) were calculated. Changes of glycosylated hemoglobin (HbA1c), tetraiodothyro-nine (T4), triiodothyronine (T3), reverse triiodothyronine (rT3), free tetraiodothyronine (FT4), free triiodothyronine(FT3) and thyroid stimulating hormone (TSH) in the two groups were determined. According to the HbA1c level, the pa-tients were divided into 〈6.5% group and≥6.5% groups. Changes in levels of thyroid hormones and glucose fluctuationwere compared, and the correlation between the two was analyzed. Results Levels of MAGE, MODD, LAGE, HbA1cand rT3 in T2MD group were significantly higher than those in the control group, while levels of T3 and FT3 were sig-nificantly lower than those in the control group (P〈0.05). Levels of MAGE, MODD, LAGE and rT3 in HbA1c level 〈6.5% group were significantly lower than those in the HbA1c level≥6.5% group, while levels of T3 and FT3 were sig-nificantly higher than those in the HbA1c level ≥6.5% group (P〈0.05). Pearman correlation analysis showed that T3and FT3 were negatively correlated with MAGE, MODD, LAGE and HbA1c (P〈0.05), and rT3 was positively correlat-ed with MAGE, MODD, LAGE and HbA1c (P〈0.05). Conclusion There are decreased levels of T3 and FT3, and in-creased levels of rT3 in patients with newly diagnosed T2MD,
