摘要
目的研究血清胱抑素C(CysC)测定对新生儿窒息后肾功能评估的临床意义。方法 100例足月窒息新生儿,根据出生后1 min阿氏评分分为轻度窒息组(64例)和重度窒息组(36例);另选取同期足月健康新生儿50例作为对照组。比较三组新生儿的血肌酐(SCr)、血尿素氮(BUN)、CysC水平,同时检测轻度窒息组和重度窒息组各个指标的异常阳性率。结果重度窒息组、轻度窒息组新生儿血清SCr、BUN、CysC水平显著高于对照组,差异具有统计学意义(P<0.05);重度窒息组新生儿血清SCr、BUN、CysC水平显著高于轻度窒息组,差异具有统计学意义(P<0.05)。重度窒息组新生儿CysC异常阳性率(44.44%)显著高于SCr(22.22%)、BUN(19.44%),差异具有统计学意义(P<0.05);轻度窒息组新生儿SCr、CysC异常阳性率显著高于BUN,差异具有统计学意义(P<0.05)。结论在新生儿窒息后肾功能评估中,可将CysC作为一项有效指标,其具有高于SCr、BUN等指标的敏感度,具有重要的临床意义。
Objective To study the clinical significance of serum cystatin C（Cys C） in the assessment of renal function after asphyxia neonatorum.Methods A total of 100 full-term asphyxia newborns were divided by Apgar score 1 min after birth into mild asphyxia group（64 cases） and severe asphyxia group（36 cases）,and concurrent 50 full term healthy newborns as control group.The levels of serum creatinine（SCr）,blood urea nitrogen（BUN） and Cys C were compared between the three groups.The abnormal positive rates of each index in mild asphyxia group and severe asphyxia group were also detected.Results Severe asphyxia group and mild asphyxia group had obviously higher serum SCr,BUN and Cys C levels than the control group,and the difference was statistically significant（P〈0.05）.Severe asphyxia group had obviously higher serum SCr,BUN and Cys C levels than mild asphyxia group,and the difference was statistically significant（P〈0.05）.Severe asphyxia group had obviously higher abnormal positive rates of neonatal Cys C（44.44%） than SCr（22.22%） and BUN（19.44%）,and the difference was statistically significant（P〈0.05）.Mild asphyxia group had obviously higher abnormal positive rates of neonatal SCr and Cys C than BUN,and the difference was statistically significant（P〈0.05）.Conclusion In the assessment of renal function after asphyxia neonatorum,Cys C can be used as an effective indicator,and it has higher sensitivity than SCr and BUN.It has important clinical significance.
