摘要
目的了解兰州市城关区社区人群甲状腺功能异常的流行病学特征及影响因素。方法按照整群随机抽样的原则,将587名符合条件者纳入研究;对研究对象进行问卷调查和甲状腺功能测定。结果甲状腺功能异常检出率为20.95%,主要以亚临床甲状腺功能减退为主(占92.68%);女性甲状腺功能异常检出率高于男性(P<0.05);随着年龄的增长,甲状腺功能异常检出率也逐渐升高(P<0.01);年龄和吸烟是甲状腺功能的重要影响因素(P<0.01);女性的甲状腺过氧化物酶抗体水平高于男性(P<0.01);甲状腺过氧化物酶抗体的水平与促甲状腺激素水平呈正相关(P=0.014)。结论兰州地区甲状腺功能异常检出率较高,应积极开展防治工作,尤其注重对亚临床甲状腺功能减退人群的筛查和干预。高龄女性为好发人群,在科学防治甲状腺功能异常的过程中应重点保护该人群。
Objective To investigate the prevalence and influencing factors of thyroid dysfunction in commu- nity residents in Chengguan District of Lanzhou. Methods According to the principle of stratified cluster random sampling, 587 eligible persons were included in the study, the subjects were investigated via question-naire and the thyroid function was measured. Results The prevalence of previous thyroid dysfunction was 2.04%, and the rate of female patients was significantly higher than that of male (P 〈 0.05), with their prevalence being 20.95%, mainly in subclinical hypothyroidism (92.68%); there is a significant difference (P 〈 0.05) in thyroid dysfunction between men and women. With the increase of age, the prevalence of thyroid dysfunction gradually increased and there was a statistically significant difference (P 〈 0.01). Age growth and smoking were important factors affecting thyroid function (P 〈 0.01). (2) The expression of thyroid peroxidase antibody (TPOAb) was higher in women than in men (P 〈 0.01), and there was no difference between differ- ent ages (P 〉 0.05). The expression of TPOAb was also positively correlated with the levels of thyroid stimu- lating hormone (TSH)(P=0.014). Conclusion The prevalence of thyroid dysfunction is high in Lanzhou area, and prevention and treatment should be carried out actively. Attention should be paid to the screening and intervention of subclinical hypothyroidism patients. Elderly women are high risk groups, and focus should be laid on protecting the crowd in the course of scientific prevention and treatment of thyroid dysfunction.
