黄芩甙对急性呼吸窘迫综合征大鼠血液流变学的影响

Effects of baicalin on changes in hemorheology of acute respiratory distress syndrome in rats

摘要 目的探讨黄芩甙对油酸致急性呼吸窘迫综合征(ARDS)大鼠血液流变学的影响。方法将健康成年雄性SD大鼠随机分为对照组、ARDS模型组和黄芩甙干预组(300 mg/kg),经股静脉注射油酸(0.12 ml/kg)复制ARDS模型,黄芩甙组同时腹腔注射黄芩甙溶液;在注射油酸后0.5及1、2、3、6、12、24 h,分别插管至腹主动脉采集动脉血进行动脉血气分析、血常规检查和血液流变学指标检测。结果模型组大鼠在注射油酸后30 min,动脉氧分压和氧合指数开始下降,氧合指数最低值平均为178 mmHg(1 mmHg=0.133 k Pa),达到了中度ARDS的诊断标准(<200 mmHg);黄芩甙治疗组的氧分压和氧合指数也有下降,但程度低于模型组,其差异有统计学意义(P<0.05);同对照组比较,模型组和治疗组PH值均有明显降低,但模型组降低的幅度更大、持续时间更长。模型组的红细胞压积在各时间点均有明显升高,血小板在0.5、1、2和3 h有明显降低且差异有统计学意义;治疗组的这两项指标与对照组差异较小。模型组的全血粘度、血浆粘度较对照组有明显升高,红细胞最大变形指数有明显降低;治疗组与模型组在有相同的变化趋势,但程度明显减轻。结论 ARDS存在明显的血液流变学异常,黄芩甙可以通过抑制微血栓形成、降低血浆粘度、降低红细胞压积、提高红细胞变形性等几个方面使血液粘度降低,从而达到改善微循环、缓解低氧血症的目的。 [ Objective ] To investigate the effect of baicalin on the changes in hemorheology of acute respiratory distress syndrome (ARDS) induced by oleic acid (OA) in rats. [Methods] Healthy adult male SD rats were randomized into 3 groups: control group,ARDS group (OA induction,0.12 mg/kg) and baicalin-treated group (300 mg/kg,intraperitoneal injection). The arterial blood samples were collected from the abdominal aorta at 0.5,1,2,3,6,12 and 24 hours after OA injection. The arterial blood gas analysis,blood routine examination and blood rheology index detection were performed. [Results] The levels of arterial oxygen partial pressure and oxygenation index in ARDS group decreased at 30 minutes after OA injection,and the lowest value of oxygenation index (178 mm Hg, 1 mm Hg=0.133 kPa) reached the diagnostic standard of moderate ARDS. In baicalin-treated group ,the levels of arterial oxygen partial pressure and oxygenation index increased, but were significantly lower than those in ARDS group (P〈0.05). The pH value in ARDS group and baicalin-treated group was lower than that in control group,while the ARDS group showed greater decrease and longer duration. In ARDS group,the hematokrit increased significantly at each time point,while the platelet count (PLT) decreased at 0.5,1,2 and 3 hours,and the differences were statistically significant. There were smaller differences in these two indexes between baicalin-treated group and control group. Compared with control group, the levels of whole blood viscosity and plasma viscosity in ARDS group significantly increased,while maximum deformability index of erythrocytes significantly decreased. The baicalin-treated showed the same change trends in these indexes as ARDS group, but the degree was obviously reduced. [ Conclusion] There is obvious abnormality of hemorheology in ARDS. Baicalin can reduce blood viscosity by inhibiting micro thrombus formation, reducing plasma viscosity, decreasing hematocrit and improving erythrocyte de- formability, so as
