摘要
目的探讨维A酸乳膏联合重组人干扰素α-2b凝胶治疗扁平疣的疗效及对T淋巴细胞亚群的影响。方法选取118例扁平疣患者,将其随机分为对照组和观察组,对照组给予维A酸乳膏治疗,观察组给予维A酸乳膏联合重组人干扰素α-2b凝胶治疗,两组患者均连续治疗6个月并随访1年,考察两组患者治疗前、后T淋巴细胞的变化,比较两组患者的临床疗效、不良反应及复发率等。结果经治疗后,观察组临床总有效率91.5%与对照组总有效率76.3%比较,差异有统计学意义(χ~2=2.5407,P<0.05);两组患者的CD+4、CD+8和CD+4/CD+8与治疗前比较,差异均有统计学意义(P<0.05),但观察组改善程度与对照组比较,差异有统计学意义(P<0.05);观察组不良反应发生率为23.7%与对照组不良反应发生率27.1%比较,差异无统计学意义(χ~2=0.0894,P>0.05);观察组复发率为5.1%与对照组复发率15.3%比较,差异有统计学意义(χ~2=1.6698,P<0.05)。结论维A酸乳膏联合重组人干扰素α-2b用于治疗扁平疣,可显著升高血清中CD+4、CD+8浓度和CD+4/CD+8的比值,增强免疫功能,提高临床治疗效果,减少复发率,且安全性较好,具有较大的临床意义。
Objective To explore the efficacy of tretinoin cream combined with recombinant human interferonα-2 bgel in the treatment of verruca plana and its influence on T lymphocyte subsets.Methods One hundred and eighteen patients with flat warts were randomly divided into the control group or observation group.The control group was given tretinoin cream,while the observation group was given tretinoin cream combined with recombinant human interferonα-2 bgel.Both groups were treated continuously for 6 months followed by1 year of follow-up.The changes of T lymphocytes before and after treatment in both groups were observed.The clinical efficacy,adverse reactions and recurrence rate were compared between the two groups.Results After treatment,the total efficiency rate was 91.5%in the observation group vs 76.3%in the control group,with statistically significant difference（χ^2=2.5407,P〈0.05）.The changes of CD＋4,CD＋8 and CD＋4/CD＋8 in both groups after treatment were statistically significant（P〈0.05）,with the degree of improvement in the observation group significantly better（P〈0.05）.The incidence of adverse reactions was 23.7%in observation group vs 27.1%in the control group;the difference was not statistically significant（χ^2=0.0894,P〈0.05）.The recurrence rate was 5.1% in observation group vs 15.3% in control group,with statistically significant difference（χ^2=1.6698,P〈0.05）.Conclusion Tretinoin cream combined with recombinant human interferonα-2 bfor the treatment of flat warts can significantly increase the levels of CD＋4,CD＋8 and CD＋4/CD＋8 in serum,improve the clinical treatment effect,reduce the recurrence rate,and has better safety,which has great clinical significance.
