Effects of protective mechanical ventilation on pulmonary function and inflammatory mediators in elderly surgical patients

摘要 目的研究保护性机械通气模式对老年患者围手术期肺功能和炎症介质水平的影响。方法将70例气管插管全身麻醉下行择期骨科手术的老年患者随机均分为保护性机械通气组(P组)及常规机械通气组(C组),检测治疗前后肺功能指标及血清炎症介质水平的变化。结果与治疗前相比,两组治疗后1秒用力呼气量(FEV1)、FEV1占用力肺活量百分比(FEV1/FVC)、PaO2和pH值均上升(P<0.05),PaCO2下降(P<0.05);P组变化较C组更明显(P<0.05)。与治疗前相比,两组治疗后血清IL-6、IL-8和TNF-α水平均下降(P<0.05),P组低于C组(P<0.05)。结论保护性机械通气模式可以改善气管插管全身麻醉下老年患者肺功能,缓解炎症反应。 Objective To explore the effects of protective mechanical ventilation on pulmonary function and inflammatory mediators in elderly patients. Methods Seventy elderly patients undergoing selective orthopedic surgery under tracheal intubation general anesthesia were randomly and equally divided into two groups of P ( with protective mechanical ventilation) and C (with conventional mechanical ventilation). The pulmonary function indexes and serum inflammatory mediator levels were detected before and after operation. Results Compared with before, the forced expiratory volume in one second (FEV1), the ratio of FEV1 to forced vital capacity (FEV1/FVC), PaO2 and pH were increased, while PaCO2 was decreased after treatment in two groups(P〈0. 05), the changes of which were more obvious in group P than those in group C(P〈0.05). Compared with before, serum levels of IL-6,IL-8 and TNF-α in the two groups were decreased after treatment (P〈0. 05), which were significantly lower in group P than those in group C(P〈0. 05). Conclusion Protective mechanical ventilation is able to improve pulmonary function and attenuate the inflammatory response in elderly surgical patients under general anesthesia.
