摘要
目的探讨脑梗死二级预防中断他汀类药物对预后和复发的影响。方法回顾分析2015年10月—2016年10月我院神经内科收治的120例脑梗死患者的病历资料,根据是否规律使用他汀类药物治疗分为2组,各60例。对照组为规律服用他汀类药物治疗患者,观察组为中断服用他汀类药物治疗患者,分析中断服用他汀类药物的原因,并对比2组患者梗死后1年的预后情况、日常生活能力、病残程度以及复发情况。结果观察组60例患者中因不良反应严重停用的占16.67%(10/60),而因其他因素停用的占83.33%(50/60)。急性期2组患者BI、mRS评分对比差异无统计学意义(P>0.05);梗死后1年,观察组BI评分较对照组低,mRS评分较对照组高,差异均有统计学意义(P<0.05);观察组复发率(18.33%)较对照组(5.00%)高,差异有统计学意义(P<0.05)。结论脑梗死二级预防中断服用他汀类药物治疗后患者的预后较差,且复发率更高。
Objective To investigate the application of statins in secondary prevention of cerebral infarction and its influence on prognosis and recurrence. Methods The clinical data of 120 patients with cerebral infarction from October 2015 to October 2016 were retrospectively analyzed, and they were divided into two groups by regular use of statins, 60 cases in each group. Control group was given regular use of statins, while observation group was given discontinued use of statins. The reasons of discontinued use of statins was analyzed. The prognosis, activity of daily living, disability degree and recurrence of patients after cerebral infarction for one-year were compared between the two groups. Results The rate of discontinuation caused by severe adverse reactions in observation group (n=60) was 16.67% (10/60), and the rate of discontinuation caused by other factors was 83.33% (50/60); The BI and mRS scores of two groups in acute phase were compared, there was no statistical difference (P〉0.05); After cerebral infarction for one-year, the BI score of observation group was lower than that of control group, while mRS score was higher than control group (P〈0.05); The recurrence rate of observation group (18.33%) was higher than control group (5.00%) (P〈0.05). Conclusion The application of statins in secondary prevention of cerebral infarction is relatively deficient, diseontinued use of statins will bring about worse prognosis and higher recurrence rate.
