使用呼吸机治疗急性有机磷农药中毒并发急性呼吸衰竭的临床效果分析 被引量:3

摘要 目的:探讨使用呼吸机治疗急性有机磷农药中毒并发急性呼吸衰竭的临床效果。方法:将2015年1月至2017年1月四川省攀枝花市第二人民医院急诊科收治的88例急性有机磷农药中毒并发急性呼吸衰竭患者分为对照组与研究组,每组各44例患者。对两组患者进行常规治疗,为研究组患者在此基础上应用Siemens Srevo-i呼吸机进行治疗,然后对比分析两组患者进行治疗前后其血氧饱和度、胆碱脂酶的水平及血气分析指标的变化情况。结果:与对照组患者相比,研究组患者在进行治疗后其血氧饱和度与胆碱脂酶的水平高于对照组患者,差异有统计学意义(P<0.05)。对两组患者进行治疗前其PaO_2、PaCO_2及pH相比较差异无统计学意义(P>0.05)。与对照组患者相比,研究组患者在进行治疗后其PaO_2较高,其PaCO_2及pH较低,差异有统计学意义(P<0.05)。研究组患者阿托品的用量少于对照组患者,其住院的时间短于对照组患者,差异有统计学意义(P<0.05)。结论:为急性有机磷农药中毒并发急性呼吸衰竭患者在采取常规急救措施的基础上使用呼吸机进行治疗可取得令人满意的效果,能有效改善其血氧饱和度、胆碱脂酶的水平及血气分析指标,减少其阿托品的用量,缩短其住院的时间。 Objective: To investigate effect of respirator for acute organophosphorus pesticide poisoning in patients with acute respiratory failure.Methods:Divide 88 patients with acute organophosphorus pesticide poisoning combined with acute respiratory failure were treated in our hospital from January 2015 to January 2017 in experimental group and control group, 44 cases each group. The control group were received routine treatment, based on routine group,treat the experimental group with Siemens Srevo-I ventilator. Then compare and analyses the change of oxygen saturation, cholinesterase levels and Pa CO of 2 groups. Results: The oxygen saturation and cholinesterase level in the experimental group were higher than those in the control group(P 0.05).The difference of Pa O2,Pa CO2 and p H of 2 groups has no statistical significant(P 0.05). The using time of atropine and duration of hospital stays in the experimental group was lower than that in the control group(P 0.05). Conclusion:Treat acute organophosphorus pesticide poisoning in patients with acute respiratory failure will achieve a satisfied effect, and will improving oxygen saturation, cholinesterase level and the index of blood gas analysis, decrease dose of atropine, shorten length of stay of patients.
