摘要
目的探讨经阴道彩色多普勒超声(TVCDS)联合血浆肿瘤标志物CA125检测在子宫内膜癌(EC)诊断中的应用价值。方法选择2013年2月至2016年12月青海省妇幼保健院EC患者225例,给予患者TVCDS联合CA125检查,与手术病理诊断结果对比,分析EC患者声像图特征及TVCDS联合CA125对子宫内膜癌的诊断价值。结果 TVCDS检查见子宫增大,内膜增厚,病灶呈不均匀低回声,病灶局限于内膜层,部分可见浸润,也可伴宫腔或盆腔积液。彩色多普勒血流显像(CDFI)检查病灶可见血流信号。TVCDS联合CA125检测灵敏度、特异度、准确性显著高于TVCDS与CA125检测(χ~2=8.22~168.98,均P<0.05),TVCDS检查灵敏度、特异度、准确性显著高于CA125检测(χ~2分别为34.63、4.20、37.26,均P<0.05)。结论 TVCDS检查EC具有独特的超声表现,TVCDS联合CA125检测诊断EC灵敏度、特异度与准确性均较高,具有较高的临床诊断价值。
Objective To discuss the value of transvaginal color Doppler sonography（TVCDS）combined with serum tumor markers CA125 testing in diagnosis of endometrial cancer（EC）.Methods Totally 225 patients with EC selected from Maternal and Child Health Hospital of Qinghai accepted TVCDS and CA125 testing.The results were compared with surgical and pathological finding.Ultrasonographic characteristic of EC patients and diagnostic value of TVCDS combined with CA125 testing were analyzed.Results TVCDS examination showed enlarged uterus,thickened endometrium and uneven hypoechoic lesions.The lesions were confined to the intima,partly infiltrated and accompanied by uterine cavity or pelvic effusion.CDFI showed blood flow signals in the lesions.The sensitivity,specificity and accuracy of TVCDS combined with CA125 testing were significantly higher than TVCDS or CA125（χ~2 value ranged 8.22-168.98,all P〈0.05）.The sensitivity,specificity and accuracy of TVCDS were significantly higher than those of CA125（χ~2 value was 34.63,4.20 and 37.26,respectively,all P〈0.05）.Conclusion TVCDS reveals unique sonographic appearance in examining EC.The sensitivity,specificity and accuracy of TVCDS combined with CA125 testing in diagnosis of EC are higher,and it has high clinical diagnostic value.
