摘要
目的:探讨不同部位骨密度(BMD)与胸主动脉钙化(TAC)之间的相关性。方法:纳入2014年2月到2016年10月所有在中山大学孙逸仙纪念医院住院的符合入选标准的骨质疏松(OP)患者共78例,纳入同期临床基本资料匹配的非骨质疏松(NOP)患者50例。收集患者的临床资料以及血清生化指标,通过CT扫描和Agatston积分评价TAC程度,采用双能X线骨密度仪测量腰椎(L1-L4)、左侧股骨颈、左侧全髋的BMD,并分析不同部位BMD与TAC的相关性。结果:与NOP组相比,OP组TAC发生率明显升高(86.4%比64.7%,P<0.05),且TAC积分更高[284(8,1429)比46(0,784),P<0.05];OP组的体重指数(BMI)和血清白蛋白(ALB)较低(P<0.05)。相关分析发现,腰椎、左侧股骨颈、左侧全髋的BMD均与BMI、ALB呈正相关(P<0.05),与年龄呈负相关(P<0.01)。股骨颈BMD与TAC有显著相关性(r=-0.248,P<0.05)。结论:股骨颈BMD与TAC呈负相关,股骨颈BMD可能为血管钙化的重要预测因子。
Objective:To investigate the association between bone mineral density(BMD)in different sites andthoracic aorta calcification(TAC).Methods:From February 2014 to October 2016,a total of 128 subjects,including78 patients with osteoporosis and 50 patients with non-osteoporosis,were enrolled in Sun Yat-sen Memorial Hospital ofSun Yat-sen University.Clinical data and serum biochemical parameters were collected from the database.According tothe Agatston method,TAC was assessed by CT scan.BMD in different sites,including lumber vertebras(L1-L4),femo-ral neck,and total hip,was measured with dual energy X-ray absorptiometry.The correlation between BMD in differentsites and TAC was analyzed.Results:Compared with non-osteoporosis group,the incidence of TAC was significantly in-crease in osteoporosis group(86.4% vs 64.7%,P0.05).Furthermore,the TAC score in osteoporosis group was higherthan those in non-osteoporosis group[284(8,1429)vs 46(0,784),P0.05].Besides,the body mass index(BMI)and serum albumin(ALB)was lower than those in non-osteoporosis(P0.05).The BMD of lumber vertebras,femoralneck and totle hip were positive correlation with BMI and ALB respectively(P0.05),and was negative correlation withage(P0.01).The BMD of femoral neck is significantly related to the TAC score(r=-0.248,P0.05).Conclusion:The BMD of femoral neck was negative correlation with TAC.It may be an important index to predict the severity of vas-cular calcification.
