给药时间对口腔鳞癌患者DCF方案所致毒性的影响

摘要 目的探讨不同给药时间对口腔鳞癌患者DCF方案所致毒性的影响,以期更好地指导临床应用。方法选择2016年3月~2017年8月于我院就诊并经病理学诊断为口腔鳞状细胞癌的患者32例,随机分为两组。实验组每天大约10∶00左右输注氟尿嘧啶,仅第1天18∶30左右输注多西他赛和顺铂。对照组第1天早餐后输注多西他赛、顺铂,每天使用微量输液泵输注氟尿嘧啶。统计第7天时患者的血液指标及不良反应发生情况,根据WHO化疗药物毒性反应分度标准进行分级。结果两组患者在基线时各项指标均无显著差异。化疗开始后第7天与基线时对比,所有患者均出现中性粒细胞减少、贫血、恶心等不良反应,而血小板减少、肌酐增加、低钾血症等不良反应较少。实验组患者中性粒细胞减少、恶心、呕吐的发生率低于对照组,差异有统计学意义(P<0.05),两组发热性中性粒细胞减少、贫血、口腔黏膜炎的发生率无显著差异(P>0.05)。结论经过调整的日间使用氟尿嘧啶及多西他赛和顺铂晚间给药的DCF化疗方案可更有效地减少化疗引起的中性粒细胞减少、恶心、呕吐等不良反应。 Objective To investigate the effects of different administration times on the toxicity of DCF regimen in order to better guide the clinical application. Methods 32 patients with oral squamous cell carcinoma were selected from March 2016 to August 2017 in our hospital. The patients were randomly divided into two groups. The experimental group was infused with fluorouracil around 10∶00 and only docetaxel and cisplatin were infused around 18∶30 on the first day. The control group received docetaxel and cisplatin after breakfast on the first day. Fluorouracil was infused using a micro-pump per day. The patient's blood parameters and adverse reactions were compared on the seventh day, the classification was determined according to the WHO chemotherapeutic toxicity classification criteria. Results There was no difference in the data between the two groups at baseline. On the 7th day after the starting of chemotherapy, compared with baseline data of patients, all patients showed several adverse reactions such as neutropenia, anemia and nausea, while a few adverse reactions such as thrombocytopenia, creatinine increase and hypokalemia occurred. The neutropenia, nausea, vomiting, several adverse reactions of patients in the two groups were statistically significant (P〈0.05), the patients in experimental group improved significantly. The patients with febrile neutropenia, anemia, oral mucositis in experimental group also improved, however, no statistical difference was found (P〉0.05). Conclusion Adjusted daytime use and evening administration of DCF chemotherapy can be more effective in reducing chemotherapeutic-induced neu-tropenia, nausea, vomiting and other toxic reactions, more researches to achieve better clinical use and reduce toxicity is needed.
