摘要
目的:观测胃转流术(Roux-en-Y,RYGB)后2型糖尿病(type Ⅱ diabetes mellitus,T2DM)模型大鼠肩胛棕色脂肪组织(brown adipose tissue,BAT)的改变,为RYGB治疗T2DM提供理论依据及其可能的机制。方法:50只SD大鼠随机分成糖尿病手术组(A=10),假手术组(B=10),糖尿病对照组(C=10),健康对照组(D=10),其余大鼠备用。糖尿病大鼠造模:高糖高脂饲料喂养14d后,给予其腹腔内注射小剂量链脲佐菌素(Streptozocin,STZ)30mg/kg进行造模,比较各组大鼠手术前后体质量(body mass,BM)、空腹血糖(fasting plasma insulin,FPG)、空腹血清胰岛素(fasting plasma insulin,Fins)、肩胛棕色脂肪组织重量及BAT细胞平均半径。结果:A组大鼠糖尿病指标较B、C组大鼠明显改善,胰岛素敏感指数显著增高;手术组大鼠较糖尿病对照组及假手术组大鼠的棕色脂肪组织重量及棕色脂肪细胞平均半径明显增大,差异具有统计学意义(P<0.01),手术组大鼠与健康对照组大鼠,糖尿病对照组大鼠与假手术组大鼠间比较无显著差异(P>0.05)。结论:RYGB能在降低糖尿病大鼠体质量及胰岛素抵抗(insulin resistance,IR)的同时促进大鼠肩胛棕色脂肪组织增多,RYGB可能通过促进大鼠体内白色脂肪棕色化的途径实现改善机体IR。
Objective:To observe the changes of brown adipose tissue( brown adipose,tissue,BAT) type 2 diabetes mellitus( type Ⅱ diabetes mellitus,T2 DM) in a rat model after gastric bypass( Roux-en-Y,RYGB) so as to provide a theoretical basis for the treatment of RYGB T2 DM and its possible mechanism.Methods:50 SD rats were randomly divided into diabetes operation group( A = 10),sham operation group( B = 10),diabetic control group( C = 10),healthy control group( D = 10),and other rats.Diabetic rats were given a high-fat diet for14 days,and intraperitoneal injection of streptozotocin( Streptozocin,STZ) 30 mg/kg modeling.The body mass of rats before and after surgery( body mass,BM),fasting blood glucose( fasting plasma,insulin,FPG),serum insulin fasting( fasting plasma insulin,Fins),the brown adipose tissue weight and average radius of BAT cells were compared.Results:The diabetic index in group A were significantly improved compared with group B and C,and the insulin sensitivity index also increased significantly.The brown adipose tissue weight and average radius of brown fat cells in operation group increased significantly compared with the diabetic control group and sham operation group.The difference was statistically significant( P < 0.01).There was no significant difference among operation group rats and healthy control rats,diabetic rats in control group and sham operation group rats( P > 0.05).Conclusion:RYGB can reduce body weight and insulin resistance in diabetic rats( insulin,resistance,IR),and promote adipose tissue of rats with brown scapular increased at the same time.RYGB may improve body IR through the way of promoting white fat brown in rats.
