胰岛素降解酶基因rs11187007位点多态性与妊娠期糖尿病患者胰岛素抵抗、全身炎症反应的相关性 被引量:5

Correlations between insulin degrading enzyme gene rs11187007 site polymorphism and insulin resistance and systemic inflammatory response in patients with gestational diabetes mellitus

摘要 目的探讨胰岛素降解酶基因rs11187007位点单核苷酸多态性与妊娠期糖尿病(GDM)遗传易感性及患者胰岛素抵抗、全身炎症反应的相关性。方法将2015年1月~2017年1月在该院住院分娩的186例GDM孕妇和158例葡萄糖耐量正常的孕妇分别纳为观察组和对照组,检测计算患者胰岛素抵抗指数(HOMA-IR)、胰岛β细胞功能指数(HOMA-β)、外周血C-反应蛋白(CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平,并采用限制性片段长度多态性聚合酶链反应(PCR-RFLP)技术对胰岛素降解酶基因rs11187007位点进行基因分型,开展相关分析。结果观察组胰岛素降解酶基因rs11187007位点AG基因型频率明显高于对照组,AA基因型频率明显低于对照组,差异有统计学意义(P<0.05)。AG基因型孕妇HOMA-IR、外周血CRP、IL-2及TNF-α水平均明显高于AA和GG基因型孕妇(P<0.05),HOMA-β明显低于AA和GG基因型孕妇(P<0.05)。二元Logistic回归分析显示,AG基因型是GDM的独立危险因素(OR=3.800,P<0.05)。结论胰岛素降解酶基因rs11187007位点AG基因型在GDM发病过程中有一定作用,且与胰岛素功能异常、全身炎症反应有关,可能是GDM的易感基因。 Objective To explore the correlations between insulin degrading enzyme gene rs11187007 site polymorphism and genetic susceptibility of gestational diabetes mellitus(GDM),insulin resistance,systemic inflammatory response of GDM patients.Methods A total of 186 pregnant women with GDM and 158 pregnant women with normal glucose tolerance were selected from Chengdu Women Children's Central Hospital from January 2015 to January 2017 as observation group and control group,respectively.HOMA-IR,HOMA-β,C-reactive protein(CRP),interleukin-6(IL-6),and tumor necrosis factor-α(TNF-α) in peripheral blood were detected.Polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP) was used for genotyping of insulin degrading enzyme gene rs11187007 site and related analysis.Results The frequency of AG genotype of insulin degrading enzyme gene rs11187007 site in observation group was statistically significantly higher than that in control group,while the frequency of AA genotype in observation group was statistically significantly lower than that in control group(P 〈0.05).The levels of HOMA-IR,CRP,IL-2,and TNF-α in peripheral blood of pregnant women with AG genotype were statistically significantly higher than those of pregnant women with AA and GG genotypes(P 〈0.05).HOMA-β in pregnant women with AG genotype was statistically significantly lower than those in pregnant women with AA and GG genotypes(P〈0.05).Binary logistic regression analysis showed that AG genotype was an independent risk factor for GDM(OR=3.800,P〈0.05).Conclusion AG genotype of insulin degrading enzyme gene rs11187007 site may play an important role in pathogenesis of GDM,and it may be related to abnormal insulin function and systemic inflammatory response,which may be a susceptible gene of GDM.
