摘要
目的探讨血清25-羟维生素D(25OHD)、甲状旁腺激素(PTH)与老年骨质疏松患者病情程度的关系。方法选取2015年1月至2016年1月该院收治的160例骨折患者。根据腰1~4椎、全髋关节、股骨颈任意部位的骨密度T值将骨折患者分为正常组53例、骨量减少组63例、骨质疏松组44例,比较三组血清25OHD、PTH、骨密度T值、血清骨钙素蛋白(BGP)、骨碱性磷酸酶(ALP)水平。结果正常组、骨量减少组、骨质疏松组的血清PTH测定值差异均无统计学意义(P>0.05);正常组的血清25OHD水平高于骨量减少组、骨质疏松组(P<0.05),正常组的血清BGP、ALP水平低于骨量减少组、骨质疏松组(P<0.05);骨量减少组的血清25OHD水平高于骨质疏松组(P<0.05),骨量减少组的血清BGP、ALP水平低于骨质疏松组(P<0.05)。正常组的腰1~4椎、全髋关节、股骨颈骨密度值均高于骨量减少组、骨质疏松组(P<0.05),骨量减少组的腰1~4椎、全髋关节、股骨颈骨密度值高于骨质疏松组(P<0.05);骨质疏松患者的血清25OHD与腰1~4椎、全髋关节、股骨颈骨密度值均呈正相关关系(P<0.05)。结论骨质疏松患者的血清25OHD水平显著降低,并且与患者骨密度水平呈正相关。
Objective To investigate the relationship between serum 25- hydroxy vitamin D (25OHD) and parathyroid hor-mone (PTH) with the severity in elderly patients with osteoporosis. Methods A total of 160 pauents with fractures treated in this hospital from January 2015 to January 2016 were selected and divided into the normal bone mass group(53 cases) ,osteopenia group (63 cases) and osteoporosxs group(44 cases) according to the bone mineral density (BMD) in the lumbar vertebra 1--4,total hip and femoral neck any position. Then the serum levels of 25OHD,PTH ,T value of BMD, serum osteocalcin (BGP) and bone alkaline phosphatase (ALP) were compared among 3 groups. Results The serum PTH detection value had no statistically significant difference among the normal group,osteopenia group and osteoporosis group (P〉0.05). The serum 25OHD level in the normal group was higher than that in the osteopenia group and osteoporosls group (P〈O. 05) ; the serum BGP "and ALP levels in the normal group were significantly lower than those in the osteopenia group and osteoporosis group (P〈0.05). The level of serum 25OHD in the osteopenia group was higher than that in the osteoporosis group (P〈0.05). The level of serum BGP and ALP in the osteopenia group was significantly lower than that in the osteoporosis group (P〈0.05). The BMD value of the lumbar vertebra 1--4,total hip and femoral neck in the normal group was significantly higher than that in the steopenia group and osteoporosis group (P〈0. 05). There was a significant positive correlation between serum 25OHD and the BMD value in lumbar vertebra 1--4, total hip joint and femoral neck (P〈0.05). Conclusion The level of serum 25OHD is significantly decreased in the patients with osteoporosis,and is positively correlated with the BMD level.
