两种尿道成形术修复尿道下裂术后冠状沟尿瘘的临床疗效对比

摘要 目的对比尿道板纵切卷管尿道成形术(TIP术)与尿道口血管基底皮瓣尿道成形术(Mathiue术)两种手术方法修复尿道下裂术后冠状沟尿瘘的临床疗效。方法选取行尿道下裂术后冠状沟尿瘘的68例患儿作为研究对象,随机分为两组。A组36例给予TIP术+睾丸鞘膜覆盖术治疗,B组32例给予Mathiue术+睾丸鞘膜覆盖术治疗,比较两组的手术效果。结果两组患儿均顺利完成手术。B组的术后并发症发生率为12.50%,显著低于A组的36.11%(P<0.05)。结论 Mathiue术式治疗尿道下裂术后冠状沟尿瘘患儿的手术效果优于TIP术式,术后并发症少,加用鞘膜覆盖可为手术提供更有效的保障。 Objective To compare the curative effects of tubularized incised plateurethroplasty (TIP) and urethroplasty of the vascular basal flap of the urethral orifice (Mathiue) in repairing coronal furrow fistula after hypospadias operation. Methods 68 cases of children with coronary sulcus fistula after hypospadias operation were selected as research objects, and randomly divided into two groups. 36 cases of group A were treated with TIP and perididymis covering, and 32 cases of group B were treated with Mathiue and perididymis covering. The surgical treatment effects of two groups were compared. Results The operations of all cases were successfully completed. The incidence of postoperative complications of group B was 12.50%, significantly lower than 36.11% of group A (P 〈0.05). Conclusions Mathiue operation in repairing coronal furrow fistula after hypospadias operation has better surgical effect than TIP operation, with fewer postoperative complications; Combining with perididymis covering can provide more effective protection for the operation.
