摘要
目的:观察高度严重不足时经牙槽嵴顶行上颌窦底骨块内嵌提升术同期植入种植体的效果及临床疗效。方法:选取2014年1月至2016年12月我院口腔科收治的128例(188颗)上颌后牙缺失患者,依据随机数字表法将患者分为经牙槽嵴顶提升上颌窦底骨块外提升术同期植入种植体治疗组(上颌窦外提升组,n=64,94颗)和经牙槽嵴顶提升上颌窦底骨块内嵌提升术同期植入种植体治疗组(上颌窦内提升组,n=64,94颗),治疗后对两组患者的临床疗效、种植位点骨高度、骨宽度、剩余牙槽骨高度(RBH)、种植体稳定系数(ISQ)、术后并发症发生情况进行统计分析。结果:上颌窦内提升组患者的治疗成功率93.75%(60/64)显著高于上颌窦外提升组71.88%(46/64),差异有统计学意义(P<0.05);上颌窦内提升组患者的种植位点骨高度、骨宽度、剩余牙槽骨高度(RBH)、种植体稳定系数(ISQ)均显著高于上颌窦外提升组,差异均有统计学意义(P<0.05);上颌窦内提升组患者的术后并发症发生率6.25%(4/64)显著低于上颌窦外提升组15.63%(10/64),差异有统计学意义(P<0.05)。结论:高度严重不足时经牙槽嵴顶提升上颌窦底骨块内嵌提升术同期植入种植体的临床效果较上颌窦外提升好,更能有效提升患者的治疗成功率、种植位点骨高度、骨宽度、剩余牙槽骨高度、种植体稳定性,降低患者的术后并发症发生率,安全有效。
Objective:To observe the height of a serious shortage of top of maxillary sinus floor lifting and clinical effect of bone embedded with simultaneous implant placement effect after alveolar ridge.Methods: From January 2014 to December 2016 in our hospital department of stomatology treated 128 cases(188 teeth) of maxillary posterior teeth missing patients, according to random number table method, the patients were divided into the alveolar crest of maxillary sinus floor augmentation with simultaneous bone block implant treatment group(maxillary sinus ascension group, n=64, 94 teeth) the crest of maxillary sinus floor augmentation with simultaneous bone embedded implant treatment group(maxillary sinus floor elevation group, n=64, 94 teeth), the clinical curative effect of two groups of patients after treatment, implant sites of bone height, bone width, residual alveolar bone height(RBH), implant stability coefficient(ISQ), operation the complications were analyzed. Results: The maxillary sinus augmentation patients in the treatment group, the success rate of 93.75%(60/64) was significantly higher than that of maxillary sinus lift group 71.88%(46/64), the difference was statistically significant(P 0.05), group of patients with maxillary sinus lift. Implant sites of bone height, bone width, residual alveolar bone height(RBH), implant stability quotient(ISQ) were significantly higher than that of maxillary sinus lift group, the differences were statistically significant(P0.05), maxillary sinus elevation in patients with postoperative complication rate was 6.25%(4/64) was significantly lower than that of maxillary sinus lift(10/64), a group of 15.63% statistically significant difference(P0.05). Conclusion: The height of a serious shortage of the crest of the ridge line of the maxillary sinus bone embedded to enhance clinical effect with simultaneous implant placement in the maxillary sinus lift, can effectively improve the patient's treatment success rate
