摘要
目的探讨心力衰竭患者医院肺部感染病原菌分布特点、耐药性及相关因素。方法选自医院2014年1月-2016年12月期间收治的心力衰竭患者718例,采集患者痰液标本,分离培养检定细菌并进行药敏试验;调查肺部感染的相关因素。结果 718例心力衰竭患者发生医院肺部感染患者70例,发生率为9.75%;共培养分离病原菌83株;其中革兰阴性菌48株占57.83%,革兰阳性菌27株占32.53%,真菌8株占9.64%;主要革兰阴性菌对头孢曲松、头孢唑啉耐药率较高,均在75.00%以上;主要革兰阳性菌对庆大霉素、青霉素、红霉素耐药率较高,均在80.00%以上;年龄、住院时间及合并糖尿病是心力衰竭患者肺部感染的相关因素(P<0.05)。结论心力衰竭患者医院肺部感染病原菌以革兰阴性菌为主,年龄、住院时间和合并糖尿病为心力衰竭患者肺部感染的相关因素。
OBJECTIVE To investigate the distribution and drug resistance of nosocomial infections in patients with heart failure and its influencing factors. METHODS A total of 718 patients with heart failure in the hospital from Jan. 2014 to Dec. 2016 were selected. Sputum samples were collected, bacteria were isolated and cultured, and drug sensitivity test was performed, Influencing factors of pulmonary infections were analyzed. RESULTS Totally 70 cases of pulmonary infections were occurred in 718 patients, and the incidence rate was 9.75%. A total of 83 strains of pathogenic bacteria were isolated, including 48 strains of gram-negative bacteria, accounting for 57.83%, 27 strains of gram-positive bacteria, accounting for 32.53%, and 8 strains of fungi, accounting for 9.64%. The gram-negative bacteria had high resistant rates of ceftriaxone and cefazolin, which were greater than 75.00%. The gram positive bacteria had high resistant rates to gentamicin, Penicillin and erythromyein, which were greater than 80.00 %. The age, hospitalization time and diabetes were related factors of pulmonary infections in patients with heart failure (P〈0.05). CONCLUSION The gram-negative bacteria are the main pathogens of nos- ocomial infections in patients with heart failure, and age, hospitalization time and diabetes mellitus are related fac- tors for pulmonary infections.
