摘要
目的探讨节拍化疗对局部晚期非小细胞肺癌(NSCLC)患者肿瘤内微血管的影响。方法将57例初治局部晚期(ⅢB~Ⅳ期)NSCLC患者分为对照组28例和研究组29例,两组均使用顺铂及长春瑞滨行化疗,对照组采用传统化疗方案,研究组采用节拍化疗方案。两组均于化疗前及化疗2周期后行CT扫描灌注成像,根据时间-密度曲线计算及比较两组NSCLC病灶的血容量(BV)、血流量(BF)及肿瘤局部血管表面通透性(PS)。结果两组化疗前BF、BV及PS值比较,差异均无统计学意义(P>0.05)。化疗2个周期后,两组BF值均较治疗前下降,且研究组化疗后BF值低于对照组(均P<0.05);但两组化疗前后BV及PS值比较,差异均无统计学意义(均P>0.05)。结论与常规化疗方案比较,节拍化疗治疗局部晚期NSCLC可更有效地减少肿瘤局部BF,但对肿瘤局部BV及血管通透性无明显影响。
Objective To investigate the effect of metronomic chemotherapy on the intratumoral microvessel in patients with locally advanced non-small cell lung cancer(NSCLC). Methods Fifty-seven patients newly diagnosis as locally advanced NSCLC of stage Ⅲ B to Ⅳ were divided into control group(28 cases) and study group(29 cases). Besides the chemotherapy with cisplatin and vinorelbine performed in the two groups,the control group was treated with the traditional chemotherapy regimen,and the study group with metronomic chemotherapy. The two groups underwent CT perfusion imaging before chemotherapy and after two sessions of chemotherapy. The blood volume(BV),blood flow(BF) and locally vascular permeability of surface(PS) of NSCLC tumors were calculated using the time-density curve and were compared between the two groups. Results There were no significant differences in BF,BV or PS value between the two groups before chemotherapy(P 0. 05). After two sessions of chemotherapy,the BF value decreased in the two groups compared to the value before treatment,and the BF value of the study group after chemotherapy was lower than that of the control group(all P 0. 05),however,there were no significant differences in BV or PS value between the two groups before and after chemotherapy(all P 0. 05). Conclusion Compared to the conventional chemotherapy regimen,the metronomic chemotherapy is more effective in reducing the local BF of tumor,but has no significant effects on the local BV and vascular permeability in the treatment of locally advanced NSCLC.
