摘要
目的分析经尿道精囊镜技术在精囊疾病诊断治疗的临床效果。方法选择2015年6月至2017年3月期间因精囊疾病于本院就诊的患者共30例为研究对象，先常规治疗，未愈患者选择经尿道精囊镜手术进行治疗，观察患者的痊愈情况和随访3个月内的预后情况。结果26例予以常规抗炎对症治疗好转，4例未愈患者在此基础上行精囊镜检查及镜下治疗。1例精囊镜下见射精管结石，术中钬激光击碎冲出；1例右侧精囊壁炎症表现，予以烧灼；1例见精阜肿大充血，精囊内未见异常；1例精阜内未发现射精管口，精阜壁炎症改变，予以钬激光烧灼。精囊镜手术时间30～95min，平均手术时间（47．1±10．5）min；出血量19-50ml，平均（34．4±8．5）ml。术后随访3个月，随访患者30例，无失访病例，1例患者于术后2d出现睾丸疼痛，诊断为急性附睾炎，经抗炎治疗5d后症状减轻。所有患者均无尿道损伤、逆行射精、直肠损伤等并发症，随访时间内未发现血精复发。结论经尿道精囊镜技术可提高精囊疾病诊断准确性，提高保守治疗无效的精囊疾病的治疗效果，值得临床推广应用。
Objective To analyze the effect oftransurethral seminal vesiculoscopy in the diagnosis and treatment of seminal vesicle diseases. Methods A total of 30 patients with seminal vesicle disease treated at our hospital from June, 2015 to March, 2017 were recruited as study objects and received conventional treatment. The unhealed patients were treated with transurethral seminal vesiculoscopy. The patients＇ recovery and prognosis were observed after 3 months follow-up. Results 26 cases were cured by conventional anti-inflammatory treatment, while 4 unhealed patients were treated with seminal vesicle examination and endoscopic treatment. Ejaculatory duct stones were found in 1 case and were smashed out by holmium laser during the operation. Right seminal wall inflammation was found in 1 case and burned by holmium laser. Intumescentia and hyperemia was found 1 case with normal seminal vesicle. Ejaculatory duct orifice could not be found in 1 case with seminal wall inflammation and was burned by holmium laser. The seminal vesicle operation time was 30-95 min, with an average of （47.1±10.5） rain. The bleeding volume was 19-50 mll with and average of （34.4±8.5） ml. All the 30 patients were followed up 3 months after the operation; 1 patient got testicular pain 2 d after the surgery and was diagnosed with acute epididymitis and the symptoms were relieved after 5 d anti-inflammatory treatment. All the patients had no urethral injury, retrograde ejaculation, rectat injury, ok u v found within the follow-up period. Conclusions Transurethral seminal vesiculoscopy can improve the diagnostic accuracy and clinical effect of seminal vesicle diseases with failures of conventional treatment, so it is worth being clinically generalized.
