摘要
目的探讨高位与低位隐睾不同的手术方式。方法将2015年2月~2016年2月我院接诊的35例隐睾患儿纳入本研究,高位隐睾在腹腔镜腹腔分离精索后将睾丸拉至阴囊固定,低位隐睾在腹腔镜探查关闭内环口后,将睾丸拉至阴囊并于阴囊开口固定睾丸,记录不同隐睾位置患儿手术情况以及随访期间不良情况。结果所有患儿手术均成功,无1例术中转开放式手术,不同隐睾位置患儿手术情况比较,差异无统计学意义(P>0.05);所有患儿术后切口均愈合良好,阴囊褶皱能够基本掩盖手术瘢痕,外观形态满意,随访10~17个月睾丸发育良好。不同隐睾位置患儿随访期间并发症发生率均较低,相互比较,差异无统计学意义(P>0.05)。结论不同的隐睾位置可采取不同的手术方式,腹腔镜手术治疗隐睾创伤小、术后恢复快、并发症少、睾丸发育良好,可根据不同隐睾位置选择合适操作方法。
Objective To investigate the different operation methods of high and low cryptorchidism. Methods 35 cases of cryptorchidism in our hospital from February 2015 to February 2016 were included in this study. After laparoscopic separation of the spermatic cord, the testis was pulled to the scrotum for high cryptorchidism, and the testis was pulled to the scrotum and the testis was fixed at the opening of the scrotum after laparoscopic exploration of the inner ring for low cryptorchidism. The operation condition of children with different cryptorchidism positions and the adverse condition during follow-up were recorded. Results All the operations were successful and there were no conversions to open surgery. There was no significant difference in the operation of children with different cryptorchidism positions （P 〉0.05）; All the incisions healed well. Scrotal folds could basically cover the scar of operation. The shape of the scrotum was satisfactory. The testis developed well after 10-17 months of follow-up. The incidence of complications in children with different cryptorchidism positions during follow-up was low, and there was no significant difference between them （P 〉0.05）. Conclusion Different positions of cryptorchidism can be treated with different methods of operation. Laparoscopic surgery for cryptorchidism is minimally invasive, fast postoperative recovery, less complications, good testicular development, can choose appropriate operation methods according to different cryptorchidism position.
