摘要
目的探讨浆细胞性乳腺炎和肉芽肿性乳腺炎的临床特征及临床治疗对策分析。方法选择2015年6月~2017年6月自愿到科室治疗的浆细胞性乳腺炎或肉芽肿性乳腺炎患者88例作为研究对象,查阅患者病例记录并比较不同病变部位、乳头内陷、肿块情况、细菌培养及泌乳素水平等临床特征;随机数字表法分为对照组(n=44例)和观察组(n=44例)。对照组采用三联抗菌药物(异烟肼、利福平、乙胺丁醇)治疗,观察组联合抗炎及类固醇激素治疗(广谱抗生素及甲基强的松龙),分析两种不同疾病的临床特征及临床治疗效果。结果浆细胞性乳腺炎以中央型为主,肿块以单个肿块为主;而肉芽肿性乳腺炎以周围型为主,肿块两个以上;浆细胞性乳腺炎与肉芽肿性乳腺炎局部均表现为乳房肿块、疼痛、发红、局部肤温升高;浆细胞性乳腺炎与肉芽肿性乳腺炎乳头内陷发生率比较,差异无统计学意义(P>0.05);浆细胞性乳腺炎细菌培养阴性率,低于肉芽肿性乳腺炎(P<0.05);观察组治疗后3个月白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)及肿瘤坏死因子-a水平,均低于对照组(P<0.05)。观察组治疗3个月药物不良反应与对照组比较差异无统计学意义(P>0.05)。结论浆细胞性乳腺炎和肉芽肿性乳腺炎存在不同的临床特征,治疗时在三联抗菌药物基础上联合抗炎及类固醇激素治疗效果理想,值得推广应用。
Objective To investigate the clinical characteristics of plasma cell mastitis and granulomatous mastitis and clinical treatment. Methods 88 cases of plasma mastitis or granulomatous mastitis treated in our department from June 2015 to June 2017 were selected as the subjects.Patients were recorded and compared with different lesion,nipple retraction and mass,were randomly divided into control group （n=44） and observation group （n=44）.The control group was treated with triple antibiotics （isoniazid,rifampicin and ethambutol）.The control group was treated with anti-inflammatory and steroid therapy （broad-spectrum antibiotics and methylprednisolone）.The two different diseases Clinical features and clinical treatment. Results Plasma mastitis mainly in the central type,the mass to a single mass-based;and granulomatous mastitis mainly to the surrounding type,more than two lumps;plasma cell mastitis and granulomatous mastitis local （P 〉0.05）;plasma cell mastitis bacterial culture was negative （P 〉0.05）; plasma cell mastitis and granulomatous mastitis nipple retraction incidence was not statistically significant （P 〉0.05）; plasma cell mastitis bacterial culture negative （IL-1β）,interleukin-6 （IL-6） and tumor necrosis factor-a （TNF-α） levels in the observation group at 3 months after treatment （P 〈0.05）,all lower than the control group （P 〈0.05）;There was no significant difference between the observation group and the control group （P 〉0.05）. Conclusion There are different clinical features of plasma cell mastitis and granulomatous mastitis.The combination of anti-inflammatory and steroid therapy on the basis of triple antibiotics is ideal for treatment and application.
